We are excited to be expanding and developing our Wellbeing Service and are seeking to recruit a skilled Registered Nurse to support with this.
One of the key aspects of this role is the development of our offer of support to those with dementia and their families, so experience in palliative care and dementia is essential.
We are looking for candidates with exceptional interpersonal and organisational skills with an empathic and compassionate nature.
A strong ability to be forward thinking to provide an empowering approach to palliative care is essential.
You will be co-ordinating the daily delivery of our support and information wellbeing space and wellbeing at home services, working alongside our volunteer workforce and wider therapeutic team.
You will be a registered nurse with a minimum of 3 years post registration and experience in palliative care / dementia.
Main duties of the job To co-ordinate a daily handover to evaluate,monitor and plan care delivery.Assess new referrals and co-ordinate dischargesfrom the services.Provide expertise and act as a resource toWellbeing and Wellbeing at Home Teams to maintain high professional standards.Support the development of the teams HealthCare Assistants and administrators by undertaking monthly one to one supportmeetings.Undertake quality assurance visits to ensureservice delivery meets the expected standards.To promote an atmosphere, within which patientsretain their dignity, independence and individuality.To deputise for the Wellbeing and Wellbeing athome Manager when requiredTo develop services using expertise andknowledge of dementia.Promote excellent communication with otherhealth professionals/agencies involved with patient care.To facilitate discussions with patients andfamilies about end-of-life care and future needs.Supervise and participate in the induction andtraining of staff, students and volunteersBe aware of and comply with Treetops policiesand procedures.Participate in regular team meetingsTo ensure accurate data input to SystmOne,bespoke database or any paperwork to maintain accurate patient records,activity monitoring and reporting in accordance with all relevant policies andprocedures and Caldicott requirements.
