Job summary The Care Navigator is a member of the reception team.Care navigators are important members of our staff and are used to support clinical staff.
You will be trained to help patients get the right help from the right place.
You will work with the patients to ensure that they are signposted to the right support at the right time.You will offer general assistance to the Practice team and be expected to project a positive and friendly image to patients and other visitors, either in person or via the telephone.You will undertake a variety of administrative duties to assist in the smooth running of the Practice.Communication between patients, members the primary health care team, secondary care and other associated healthcare agencies is highly important to enable the smooth running of the practice and you will play a pivotal role in this.
Main duties of the job The main duties of the post will be dealing with patients in a polite and confident manner, answering the telephones, liaising with clinicians and team members, and using our Emis Web clinical system.
This role will require a sensitive, empathetic manner.Welcoming patients and visitors, in person or on the telephone.Answering and/or referring enquiries appropriatelyMaintaining confidentiality of personal information.Assisting in maintaining the smooth running of the reception office by carrying out general administrative and reception tasks.Contribute to the team effort by working effectively as a valued team member.All the necessary training and support will be given.Progression within the practice team also an option once employed.
