Job summary The Discharge to Assess pathways supports individuals to return to thecommunity from hospital with the appropriate level of support based on need.This need can include reablement to help promote the individuals to return totheir previous level of independence and wellbeing utilising Enhanced RecoveryBeds.
This multi-disciplinary approachis based within the Community Urgent Care Team Therapists, also workingalongside Adult social care partners in Focus and the Community Stroke team.
