Job summary The Discharge to Assess pathways supports individuals to return to thecommunity from hospital with the appropriate level of support based on need.This need can include reablement to help promote the individuals to return totheir previous level of independence and wellbeing utilising Enhanced RecoveryBeds.
This multi-disciplinary approachis based within the Community Urgent Care Team Therapists, also workingalongside Adult social care partners in Focus and the Community Stroke team.

Maybe required to work within North East Lincolnshire.Up to hours per week Until March 2025Salary Range 4 £24,645 - £27,202per annum, you will be appointed a fixed point within therangeThe successful applicant will co-ordinate betweenthe multi-disciplinary agencies to organise the delivery of services to supportthe individual in a timely manner whilst in an Enhanced Recovery Bed.

Person Specification Experience Essential At least 1 years recent (6-12 months) administrative experience at senior level Experience of dealing with day-to-day staffing issues Dealing with complaints Experience of, organising and attending meetings Experience of working within a multi-functional office environment Accurate manipulation of complex data/data management/audit Desirable Experience of working within a health and care setting Experience of cross agency working Budget Monitoring Experience working with Community Intermediate Care & Therapy Service Other Essential Ability to demonstrate commitment to CPG core values Flexible approach to duties and working arrangements Perform and deliver administrative function with professionalism Qualifications Essential NVQ 3 in business administration or equivalent Level 3 Computer application qualification or equivalent competency Literacy and Numeracy level 2 Desirable Supervisory qualification HNC in Business Knowledge and Skills Essential Effective communication skills both written and verbal Effective organisational / time management skills Ability to work to work accurately and to deadlines Ability to act on own initiative as well as working as part of a team Proficient in the use of Microsoft Office Applications Complex minute taking Ability to identify solutions to problems encountered within remit of work role Awareness of health and safety at work Ability to deal with conflict and difficult/challenging situations
