Job summary To work within Rapid Diagnosis Clinic (RDC) Multi-disciplinary Team (MDT) within the Betsi Cadwaladr University Health Board, be a core member of the MDT and the designated named lead nurse within the team for those patients.This role aims to support the swift diagnosis of patients with vague but serious symptoms indicative of cancer through the RDC service and will facilitate in the rapid evaluation of patients to enable a timely, definitive diagnosis to be found.
The post holder will assist the RDC Lead Clinician in processing the referrals received and facilitating the most appropriate pathway for those patients.The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job To demonstrate advanced and highly specialist practice in holistic assessment, planning, implementation and evaluation of specialist practice to patients and their families, according to changing health care needs.
This will necessitate high levels of autonomy and advocacyand include receiving direct referrals, undertaking differential diagnosis and ensuring the delivery of ongoing appropriate care, to maintain quality of life.
To define a clear care pathway, referring to other professionals and signposting to other agencies as appropriate.
To manage a specialist caseload and be responsible and accountable for delivering evidence based individualised and personalised care through assessing, planning and evaluation of all care and interventions.
To communicate highly sensitive information and provide complex psychological and emotional support to patients and their families by utilising a wide range of advanced communication and interpersonal skills.
To demonstrate skills in conflict resolution and competent negotiation skills when dealing with difficult or challenging situations.
To act as a role model demonstrating high standards of holistic care and provide clinicalleadership in specialist area.
To empower the multidisciplinary team with the skills and knowledge, to achieve the highest standards of care.
To promote clinical excellence reflecting the Nursing Strategy of the Betsi Cadwaladr University Health Board.
