Rapid Assessment & Resettlement Practitioner (Scale 6)

Who You Are You are a proactive and compassionate individual with a strong ability to assess, design, and implement Resettlement and Personal Housing Plans.
You have experience working in homelessness services, particularly around street homelessness.
You possess excellent collaborative skills, enabling you to work effectively with service users and partners to develop comprehensive housing solutions.
You are equipped with a sound understanding of welfare, housing, and homelessness policy, and have demonstrated the ability to meet performance targets.
You are adaptable, willing to work flexible hours, including shifts on nights and weekends.
You embody the values of accountability, respect, continuous learning, and collaboration.
What the Job Involves The role involves providing high-quality assessments and resettlement plans to assist individuals who have experienced street homelessness in achieving personal housing outcomes.
You will work closely with Specialist and Housing Coordinators to deliver tailored Personal Housing Plans and fulfill other homelessness duties.
A critical aspect of your role is ensuring all documentation is up-to-date and accurately input into the systems.
You will also play a part in building effective work relationships with service users and stakeholders and provide practical support tailored to the needs of these individuals.
Other duties include undertaking casework and collaborating with other professionals to support service users and ensure effective service delivery.
Compliance with the Council's policies, including safety and data management, is essential.
The job may also require you to support out-of-hours operations, contributing to the management and maintenance of the building.
