They are looking for a new member for their Adult Neighbourhood Team.
Benefits of the role: Remote/hybrid working available Great reputation and links with health partners Supportive team with great training in place Essential requirements: Hold a social work degree or equivalent qualification with at least 3 years' experience in a statutory Adult Social Care setting.Demonstrate motivation to challenge existing practices and engage with strengths-based assessment approaches.Work with adults experiencing a range of challenges, including long-term conditions, substance abuse, hoarding, dementia, and alcohol issues.Conduct social work assessments and interventions that are strength-based and person-centered, securing necessary funding in line with Reshaping Trafford Policy.Provide effective responses to safeguarding enquiries following the Care Act and local policies.Collaborate with West Neighbourhood Team to deliver high-quality care to adults with diverse needs.
