Job summary The Primary Care Network Managerrole is strategic and transformational.
The post-holder is part of the PCN leadership team,working alongside the Clinical Director to support the PCN boardmembers, to deliver the Primary Care Networks agenda.The PCN Manageris independent of member practices, actingas a source of expertise, to co-ordinate the management of all governance requirements,and to help shape and implement meaningful services in line with national andlocal policy and contract/s, supporting member practices and to achieveimproved outcomes for the local population.
The PCN Manager may also buildrelationships with other stakeholders, including local authorities, thirdsector and patient representative organisations, other local PCNs andhealthcare providers and the ICB.
Main duties of the job To play a pivotal role in the strategic development andcontinued evolution of HBB PCN to support the delivery ofhigh-quality primary care services across member practicesResponsible for the smoothrunning of network services,as agreed by the BoardTo manage, with the Clinical Director,the Networks financial processes and governanceResponsible for presenting monthly performance, finance,and quality reportsto the Network Board.To ensure the PCN is compliantwith reporting requirements and all relevantlegislationTo ensure all Board,Network and other meetings are managed as agreed by the BoardTo represent the PCN externally as agreedwith the Clinical Directorand the Network Board About us HBBPCN has four member practices* across Hanley and Bentilee,within Stoke-On-Trent.
Together the practices have a patient population of approximately 33,000.
The PCN serves an area with pockets of significant deprivation.

Strategy, Leadership and Network DevelopmentRegularly review and agree the Networks purpose and valueswith member practices, Clinical Director and Boardand ensure all services and developments are inline with these Establish and agree objectives and priorities for Network activity in line withcontract requirements, the Network Agreement, and budget availability, usingpractice and population health data to shape effective use of all resources Work with the NetworkBoard to develop,agree and regularlyreview a costed Network Business Plan that deliversrelevant services and improvement projects which can be measured and evaluated.
Explore activities that could deliver efficiencies and/or savings for member practices OrganisationalTo be responsible, with support of the PCN Administration Support, for servicesprovided through the PCN on behalf of member practices To collate and analysedata on service provision and uptake and report to the Network Board and other organisations asrequired To ensure the PCNsBusiness Continuity policyis updated regularly and routinelyshared with staff and member practices as requiredTo oversee use of any risk stratification or data analysis tools to support the PCN in meeting contractual obligations and ensuring the best health outcomesfor the PCN patient population Manage PCN meetings, ensuringthat papers required prior to meeting and minutes /action logs from those meetingsare circulated in a timely manner to Board members, the ClinicalDirector and others as appropriate.
Governance and QualityMaintain quality and ensurePCN activity complieswith all relevantlegislation and operates inline with agreed Network governance and SOP standards Ensure all PCN businessis conducted in line with the Network AgreementEnsure the PCNs compliance with all relevant GDPR and FOI requirements andother relevant legislation and professional standards such as GMC/BMAand LMC guidance.
FinanceResponsible for achieving the best impact from well-managed finances by: oensuring the accuracy of any submissions required by the ICB/other authorities to claim all funding entitlement and to ensure that all moniesdue are received promptly oreviewing and approving operational services spend, monitoring income and expenditure and identifying and reporting to the CD andNetwork Board any significant deviations from forecast oplanning and monitoringincome and cash flow to ensure that income is maximised and that funds are availableas required Present financial forecasts and budget reportsto the PCN Board as required Ensure financial risks are recognised and discussed with the Clinical Directorand Network Board members and appropriate action taken Ensure recommendations to the Boardregarding allocation of resourcesare fair and equitableManage the PCN bank account(currently a sub account within a Practice account)and all delegatedbudgets with proprietyin line with all relevant organisational procedures Liaise with accountant and theClinical Director to produce year-end accounts and present these tothe Board in line with Network Agreement.
Relationship ManagementEnsure engagement of memberpractices in accordance with the NetworkAgreement to: omaintain support for agreedPCN-wide activities o foster a cultureof collaboration to improve patientoutcomes, and resilience and capability across member practices oembed the PCNs ethosand values Develop, agree and deliveran internal and external communication strategy Represent the network at meetings in the communityand to the media as required.
