Job summary We have an exciting opportunity for an Advanced Nurse/Clinical Practitioner to join our super GP practice in Crawley, West Sussex.
We aim to ensure that the highest levels of care are delivered to all our patients, particularly in areas such as minor illnesses and acute, on the day appointments.

Main duties of the job To assess, diagnose, treat, referor signpost patients who attend surgery with undifferentiated or undiagnosedconditions relating to minor illness.
To undertake home visits,primarily for acute conditions, as assessed by your duty doctor, and within theboundaries of professional qualifications.
The post holder will use advancedclinical skills to provide education to patients, promoting self-care andempower them to make informed choices about their treatment.
About us Bridge Medical Centre comprises 4 GP Partners, a nursing team of 6, Paramedic Practitioners, an Advanced Nurse Practitioner and a vast supportive administrative team.
The Practice is part of the Crawley Care Collaborative PCN who are each members of the Alliance for Better Care Federation.Our clinicians work independently as well as part of a team collaborating with our own clinical and administrative team whilst also working closely with the Additional Roles from the PCN.
These roles comprise Clinical Pharmacists, Dieticians, First Contact Physiotherapists, Dieticians, Care Co-ordinators and Social Prescribers.We are a sociable Practice and like to socialise and celebrate our success.
Job description Job responsibilities Diagnosing and treating patients presenting with minorillness· Triage and treat patientswishing to see a health care professional, making any necessary referrals toother members of the primary health care team· Patients with problems needing referral tosecondary care should be discussed with the registered GP before making suchreferral.· Ensure clinical Practice is safeand effective and remains within boundaries of competence, and to acknowledgedlimitations.· Advise patients on generalhealth care and minor ailments, with referral to other members of the primaryand secondary health care team as necessary· To undertake home visits,primarily for acute conditions, as assessed by your duty doctor, and within theboundaries of professional qualifications.· To communicate at all levelswithin the team ensuring an effective service is delivered.· To maintain accurate, up-to-date healthcarerecords appropriate to the consultation.· Ensure evidenced-based care isdelivered to the highest standards.· Enhance own performance throughContinuous Professional Development, imparting own knowledge and behaviours tomeet the needs of the service.· To participate in the auditprocess, evaluation and implementing plans and Practice change in order to meetpatient need.· Contribute positively to the effectivenessand efficiency of the team and work colleagues.Pathological specimens and investigatory procedures· Undertake the collection of pathologicalspecimens including intravenous blood samples, swabs etc.
Perform investigatoryprocedures requested by the GPsAdministration and professional responsibilities· Participate in theadministrative and professional responsibilities of the Practice team· Ensure System 1 is kept up todate, with accurate details recorded and amended· Ensure accurate completion of allnecessary documentation associated with patient health care and registrationwith the Practice· Ensure collection andmaintenance of statistical information required for regular and ad hoc reportsand audit· Attend and participate in Practicemeetings as requiredTraining and personal development· Training requirements will bemonitored by yearly appraisal and will be in accordance with Practicerequirements.

· Participate in the education andtraining of students of all disciplines and the induction of all members of thePractice staff where appropriate· Maintain continued education by attendanceat courses and study days as deemed useful or necessary for professionaldevelopment.· If it is necessary to expand therole to include additional responsibilities, full training will be given.

Qualifications Essential Qualified Advanced Nurse/Clinical Practitioner Evidence of appropriate knowledge base.
