Job summary We are looking to recruit an experienced, motivated and enthusiastic Pharmacy Technician to join our GP practice to work with our other health care professionals in our practice.
The role will be based in Bowland Medical Practice which is part of Wythenshawe Primary Care Network.You will have a key role in supporting the pharmacists, clinicians,administration teams and patients in the delivery of high quality, innovativeand safe prescribing practices ensuring the best clinical outcomes for our patients.The expertise and resources within our practices arededicated to a person-centred approach for those who use our services and forall our employees.

Main duties of the job Thepost holder will help patients to get the best from their medicines by improving repeatprescribing processes in General Practice, and aiming to reducewasted medicines.Other duties :Respondto medication queries/shortages/recalls.Lead onmedication synchronisation to support patients and community pharmacy to managepatient medication ordering.Buildrelationships with hospitals and care homes in order to optimise patient therapy, minimise error and allow for a smoothtransition between care settings.Supportthe pharmacist and practice team with the repeat prescribing reauthorisationprocess.Participatein the induction, education, and training of new members of practice staffwhere appropriate.Participate in medication audits and support theclinical team on prescribing projects.Workautonomously and be capable of prioritising work, escalating when appropriate.

Job description Job responsibilities Respondto medication queries from patients, GPs, nurses, receptionists, and otherhealthcare professionals within the post holders expertise in a professionalmanner removing unwanted items, checking review dates, ensuring necessarymonitoring has been carried out, arranging compliance aids and investigatingout of stock medicines.
The post holdershould refer on to an appropriate clinician where necessary in accordance withpractice protocols.Support the clinical pharmacist to manage drugshortage queries and drug recalls.Liaisewith local community pharmacies and hospitals regarding prescription queries,helping to ensure a smooth transition from secondary to primary care.Buildstrong working relationships with local community pharmacies and other care providersto assist patients and provide a high-quality service.Lead onmedication synchronisation to support patients and community pharmacy to managepatient medication ordering.Understand andapply the protocol for seven or twenty-eight day prescribing when communicatingwith community pharmacy to arrange compliance aids.Buildrelationships with care home teams in South Manchester in order to optimisepatient therapy, minimise error and allow for a smooth transition between caresettings.Undertakemedicine reconciliation from hospital discharge and secondary care clinicdocumentation in accordance with practice protocols and update patientsmedical records accordingly.Have acomprehensive understanding of Repeat Dispensing (eRD) when it is appropriateto use and how to manage this type of prescribing.
Have the ability and confidence to traincurrent and new members of staff in the management of this type of prescribing.Supportthe pharmacist and practice team with the repeat prescribing reauthorisationprocess by reviewing patient requests for repeat prescriptions and reviewingmedicines reaching review dates.Work with the clinical pharmacist andprescribing co-ordinators to contribute to the review and development of repeatprescribing protocols / Standard Operating Procedures to improve the efficiencyand effectiveness of repeat prescribing systems.Provide advice to patients and the practice teamin changes to prescribing to support the improvement of prescribing safety,quality and cost effectiveness.Become familiar with the Greater ManchesterMedicines Management Group drug traffic light system for prescribing of highrisk drugs and liaise with secondary care specialists to arrange shared careagreements where needed.Developand implement computer searches to identify cohorts of patients at high risk ofharm from medicines.Participatein the induction, education, and training of new members of practice staffwhere appropriate.Have an understanding of the practicesprescribing Quality and Outcomes Framework targets and work to support thepractice to achieve these where possibleParticipate in medication audits and support theclinical team on prescribing projects.To assist in the submission of claims forpersonally administered drugs as required.Attendpractice and other meetings of relevance where appropriate.To help plan, develop and support theintroduction of new working processes within the practice to optimise thequality of prescribing.Maintainand develop professional competence and expertise, participate in the appraisalprocess and agree objectives and a personal development plan.Workautonomously and be capable of prioritising work whilst maintaining accurateand timely records, escalating when appropriate.
Be responsible for own workload to meet setdeadlines.Undertakeany other duties applicable to the post holders grade as agreed with the postholders line manager.
Person Specification Qualifications Essential Diploma Level 3 or equivalent in Pharmacy Services.
Registered with the General Pharmaceutical Council (GPhC) Must have completed or be enrolled in, be undertaking or be prepared to start an approved 18-month training pathway ( Primary care pharmacy educational pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)) Communication Essential Can communicate sensitive information in an understandable form.

Knowledge and understanding of responsibilities related to patient information and confidentiality.
Awareness of responsibilities regarding health and safety particularly when working without direct supervision Understanding of clinical governance and quality measures Recognises priorities when problem solving and identifies deviations from the normal pattern and can refer to the Pharmacist or GPs when appropriate Desirable Experience of working in Primary Care.
