Job summary The Stokes Primary Care Network (PCN) is looking for an enthusiastic and highly motivated Pharmacist to join our team.

Wehave been forward thinking and ambitious in the way that we have developed the Pharmacistrole to provide interesting and varied work that fully supports our patients.
This has included work to allow pharmacists to develop in areas that are of interest to them and to provide services that would previoulsy have been provided by GPs and long term condition nurses.
We are keen to provide development and stretch in this role and see the pharmacists as incredibly important elements of the wider clinical team.
Main duties of the job Working with our prescribing team, pharmacy technicians, pharmacists, GPs and nurses, the pharmacist will: conduct medication reviews and reconciliations; respond to safety and dashboard alerts; manage clinic letters and discharge summaries; support patients with long term conditions ( management of HRT, statins, hypertension and diabetes medication); respond to medication queries; lead on prescribing projects; support QOF, and provide specialist information to other clinicians.
About us From1 Apr 25, the Stokes PCN will be formed by the Stokes Medical Group following themerger of the 2 existign PCN Practices: Bradley Stoke Surgery and Stoke Gifford Medical Centre.
The 2 practices have worked closely over the last 5 years to share experience and ideas and develop new ways of working.
We are excited to become a new and larger organisation that will provide care to 40,000 patients with more growth expected.The Medical Group will have a team of 6x pharmacists, 3 pharmacy technicians and a prescribing team that will largely manage all medication related work.
Scale will bring opportunity to specialise further and develop additional pharmcist led patient services.The post will be based at and work from StokeGifford Medical Centre although some PCN work will be required.
Job description Job responsibilities JOB provide pharmaceutical support to the Practice tosupport safe and effective prescribing under the guidance and direction of the LeadGP.
Prescribe medication for patientswithin agreed protocols.
Provide pharmaceutical advice to patients to enablethem to get the best from their medicines.KEY RESPONSIBILITIESTo reconcile medicines following hospital dischargeand work with patients and community pharmacists to ensure patients receive themedicines they need post discharge.To provide pharmaceutical medication review andhealth education advice to patients at the designated practices by face toface, telephone or by notes-based review as agreed with the practice.Make clear and comprehensive records of anyinterventions etc in patients EMIS records so that they are available to otherpractice staff.To provide pharmaceutical input to long termconditions clinics as appropriateTo hold medication review clinics and facilitateimplementation of changes with appropriate follow-up monitoringPrint FP10MDA instalment prescriptions to ensurethey meet legal requirements following the agreed protocol (scripts to besigned by GPs and not by pharmacist).To develop and run pharmacist-led clinics to monitorthe effects of medication by checking clinical signs and symptoms wheremedicines are the main intervention.

Support processes for repeat prescriptions for carehomes that are looked after by SGMC.To participate in continuing professionaldevelopment and work towards an Independent Prescribing qualification.To contribute to public health campaigns, includingflu vaccinations, childhood immunisation and adult immunisation programmesTo act as a source of medicines information for allof the practice team and patients: around doses, side effects, adverseevents, possible alternatives (around out of stocks).To improve prescribing practice though educationalsupport for all prescribers within the Practice.Work with prescribers, local pharmacists, patientsand their carers to arrange dosette boxes / other compliance aids.To lead on areas where changes in evidence requirechanges in prescribing across patient populations where a drug iswithdrawn or indications change.To support improvements in clinical care throughpractice based audits and implementing changeTo liaise with community pharmacy regarding transferfrom care settings and the use of monitored dosage systems.To provide a telephone helpline for patients withquestions, queries and concerns about their medication.
(Repeatprescription requests will usually be signed by a GP but the pharmacist mayprescribe an item from the patients repeat if it is within their clinicalcompetence to do so and circumstances deem this the best course of action seeProtocol for Pharmacist Prescribing.
)To manage and support the repeat prescribingreauthorisation process by reviewing requests for repeat prescriptions andmedicines reaching review dates where required.To monitor adherence to the repeat prescribingpolicy ensuring repeat medication reviews are conducted at the appropriateintervalsTo maximise the potential of the appropriate use ofthe repeat dispensing service.To promote electronic prescribing services withinthe practiceTo implement drug withdrawals and alerts, MHRAalerts, aimed at improving medicines safety.Supportthe development of policies, clinical protocols and guidelines in the practiceDraftingreports summarising status on issues, appraising outcomes, and providingprogress reports for the Lead Medicines Optimisation Pharmacist and Practice GPPrescribing LeadCollateas required, qualitative and quantitative information and lead appropriateanalysis to develop and contribute to projectsAnalyse, interpret and present data tohighlight issues, risks and support decision making.Liaises with the CCG MedicinesManagement Team on prescribing related matters to ensure consistency of patientcare and benefit Person Specification Knowledge and Skills Essential Skills A good clinical knowledge Excellent verbal and written communication skills Good interpersonal and presentation skills Self-Motivated and able to work on initiative Capability to work well in a team and across multiple organisations Problem solving Ability to deal diplomatically with conflict or confrontation Ability to negotiate change and influence others Demonstrates accountability for delivering professional expertise as an individual Ability to facilitate, motivate, organise Aware of current issues in medicines optimisation Knowledge of current NHS policies / processes and structures relating to prescribing and medicines Good level of computer literacy and word processing skills Desirable Ability to facilitate, motivate, organise Aware of current issues in medicines optimisation Knowledge of current NHS policies / processes and structures relating to prescribing and medicines Experience Essential Minimum 2 years experience post registration Experience of working in primary and/or secondary care settings Previous work in a multi-disciplinary team Desirable Previous experience of using prescribing data Qualifications Essential Pharmacy Degree Registered with General Pharmaceutical Council Evidence of commitment to continuing professional development Desirable Relevant postgraduate qualification Holds or is working towards an Independent Prescriber qualification Member of the Royal Pharmaceutical Society
