In our latest 2024 survey ALL staff said that we fostered a positive and inclusive culture- GOOD salary- Sixweeks' annual leave + bank holidays pro rata- Highstaff retention-Occupational sick pay *once attained minimum period of service*- Purposebuilt building- Targetdriven Main duties of the job To be responsible for the implementationof processes for the effective management of patients with long-term conditionsusing evidence-based practice including but not limited to: Asthma, COPD,Diabetes, Hypertension, Non-Diabetic hyperglycaemia, CKD, CHD and CVA/TIA.Working as a member of the practicemultidisciplinary team, the post holder will ensure nursing services aredelivered effectively to the entitled patient population.The Practice Nurse will be responsiblefor a number of clinical areas such as infection prevention and control, healthpromotion, chronic disease management, health promotion, well women and wellman clinics, as well as actively supporting the practice management team in thereviewing and delivery of clinical policy and procedure.
About us The team consists of; 5 GP Partners, 2 Salaried GPs, 1 Managing Partner, 2 Advanced Nurse Practitioners, 2 Musculoskeletal Care Practitioners, 1 Clinical Pharmacist, 1 Pharmacy Technician, 2 Practice Nurses, 1 HCA and Social Prescribers.
The team is supported by a hardworking and friendly reception and secretarial team.
As an undergraduate teaching practice, we have close ties with Manchester and Keele University School of Medicine.
We are also a Training Practice and support GP Trainees.We are paperless utilising EMIS Web, Docman, Accurx, lab links, Lexacom and Electronic Prescribing, and are extremely high achievers in both national and local performance frameworks and enhanced services.
Our notes are fully digitised and we undertake self-care and workflow redirection.
