Job summary We are a busy dual location practice who is looking for a practice nurse as many of our current nurses are sadly retiring.We are mainly looking for a Treatment Room nurse, although any additional knowledge of chronic conditions would be an advantage.
Main duties of the job To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.Working as a senior member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population.The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.This organisation must ensure that the post holder hasaccess to appropriate clinical supervision and an appropriate named individualin the organisation to provide general advice and support on a day-to-daybasis.
