Job summary We are a busy dual location practice who is looking for a practice nurse as many of our current nurses are sadly retiring.We are mainly looking for a Treatment Room nurse, although any additional knowledge of chronic conditions would be an advantage.
Main duties of the job To be responsible for the implementation of processes for theeffective management of patients with long-term conditions using evidence-basedpractice including care for elderly and housebound patients.Working as a senior member of the practice multidisciplinary team,the post holder will ensure nursing services are delivered effectively to theentitled patient population.The Practice Nurse will be responsible for a number of clinicalareas such as infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.This organisation must ensure that the post holder hasaccess to appropriate clinical supervision and an appropriate named individualin the organisation to provide general advice and support on a day-to-daybasis.

Our demographic is a mix of families, elderly, students, professors, and a bail hostel.
We love the variety this gives us!We currently employ around 70 people, with multiple GPs, paramedics, a physician's associate, pharmacists, and of course, our wonderful reception and admin team.
Job description Job responsibilities The following are the core responsibilities ofthe Practice Nurse.
There may be, on occasion, a requirement to carry out othertasks.
This will be dependent upon factors such as workload and staffinglevels:To beinstrumental in the support of the Infection Prevention Control (IPC) lead andto work to the IPC requirements by providing audits, training, support andevidence of compliance to the practice managerTo supportin the publication of the IPC annual statement in conjunction with the practicemanagerTodevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practiceToassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practiceTodevelop, implement and embed health promotion and wellbeing programmesToimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeingToidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients healthAsrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSFAsrequired, to diagnose and manage acute and chronic conditionsProvidewound care (ulcer/Doppler, etc.)
to patientsToreview patient medications to enhance complianceToprovide guidance to patients in the use of prescribed and over-the-countermedications regarding side effects and dosagesToco-ordinate the provision of travel medicine servicesTorequest pathology services and process pathology results as requiredTooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as requiredTomaintain accurate clinical records in conjunction with extant legislationToensure SNOMED CT codes are used effectively by all members of the nursing teamToensure the appropriate maintenance and use of chronic disease registersToensure compliance with the practice chaperone policyToassist GPs with minor surgery when requiredToprioritise health issues and intervene appropriatelyTosupport the team in dealing with clinical emergenciesTorecognise, assess and refer patients presenting with mental health needsToimplement vaccination programmes for adults and childrenTo support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice)To liaise with externalservices/agencies to ensure the patient is supported appropriately (vulnerablepatients etc.
)To delegate clinical responsibilitiesappropriately (ensuring safe practice and the task is within the scope ofpractice of the individual)To support the clinical team with allsafeguarding matters, in accordance with local and national policiesTo understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriatelyTodeliver opportunistic health promotion where appropriateIn addition to the primary key responsibilities,the Practice Nurse may be requested to:Developand review audit protocols and process for the practice, effectively utilisingthe audit cycleDevelop,review and implement nursing protocols in conjunction with the partnersSupportall members of the nursing team, providing guidance when necessary, conductingappraisals and training needs analyses as requiredParticipatein local initiatives to enhance service delivery and patient careSupportand participate in shared learning within the practice in order to improvepatient careContinuallyreview clinical practices, responding to national policies and initiativeswhere appropriateParticipatein the review of significant and near-miss events applying a structuredapproach, , root cause analysis (RCA)Coordinatethe nursing team rota, ensuring sufficient staff are available to meet patientneedsLeadthe planning and implementation of changes within the nursing team, enhancingthe provision of services to patientsSupport the practice team with thereviewing and implementation of practice policies and protocols, ensuringconformance to extant legislation Person Specification Qualifications Essential - Registered with the NMC - Registered Smear taker - Degree in nursing Desirable - Experience in managing chronic illness Experience Desirable - Experience in working in primary care
