Job summary We are looking to recruit a practice nurse to join our multi-skilled, supportive team delivering evidence based high quality care to approximately 12,500 patients.
Main duties of the job To be responsible for theimplementation of processes for the effective management of patients withlong-term conditions using evidence-based practice including care for elderlyand housebound patients.
The post holder will ensure nursing services aredelivered effectively to the entitled patient Practice Nurse will be responsible for a numberof clinical areas such as infection prevention and control, health promotion, chronic disease management, healthpromotion, well women and well man clinics, as well as actively supporting thepractice management team in the reviewing and delivery of clinical policy andprocedure.
About us Jubilee Medical is an established GP practice based in Newton Aycliffe with branch sites in Shildon and Eldon Lane.
The practice is passionate about providing additional services to our local community, currently offering consultant led Paediatric and ENT clinics, out of hours access, enhanced access and home visits (including weekends and bank holidays) to all patients registered within our Primary Care Network.
We also have a dedicated team of Advanced Nurse Practitioners supporting our local care homes ensuring our elderly population receive the appropriate clinical care and monitoring.
The practice has been operating as a Covid Vaccination Site since December 2020.
Job description Job responsibilities Primary key responsibilities The following are the core responsibilities of the Practice Nurse.
There may be, on occasion, a requirement to carry out other tasks.
This will be dependent upon factors such as workload and staffing levels: To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice managerTo support in the publication of the IPC annual statement in conjunction with the practice managerTo develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practiceTo assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practiceTo develop, implement and embed health promotion and wellbeing programmesTo implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeingTo identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients healthAs required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSFAs required, to diagnose and manage acute and chronic conditionsProvide wound care (ulcer/Doppler, etc.)
to patients To review patient medications to enhance complianceTo provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosagesTo co-ordinate the provision of travel medicine servicesTo request pathology services and process pathology results as requiredTo oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as requiredTo maintain accurate clinical records in conjunction with extant legislationTo ensure SNOMED CT codes are used effectively by all members of the nursing teamTo ensure the appropriate maintenance and use of chronic disease registersTo develop, implement, embed and participate in well woman clinicsTo develop, implement, embed and participate in well man clinicsTo ensure compliance with the practice chaperone policyTo assist GPs with minor surgery when requiredTo prioritise health issues and intervene appropriatelyTo support the team in dealing with clinical emergenciesTo recognise, assess and refer patients presenting with mental health needsTo implement vaccination programmes for adults and childrenTo support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.
)To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)To support the clinical team with all safeguarding matters, in accordance with local and national policiesTo understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriatelyTo deliver opportunistic health promotion where appropriate Secondary key responsibilitiesIn addition to the primary key responsibilities, the Practice Nurse may be requested to: Develop and review audit protocols and process for the practice, effectively utilising the audit cycleDevelop, review and implement nursing protocols in conjunction with the partnersSupport all members of the nursing team, providing guidance when necessary, conducting appraisals and training needs analyses as requiredParticipate in local / national initiatives and targets to enhance service delivery and patient care (eg QOF)Support and participate in shared learning within the practice in order to improve patient careContinually review clinical practices, responding to national policies and initiatives where appropriateParticipate in the review of significant and near-miss events applying a structured approach, , root cause analysis (RCA) Lead the planning and implementation of changes within the nursing team, enhancing the provision of services to patientsSupport the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation Person Specification Clinical knowledge and skills Essential Wound care/removal of sutures and staples ECGs Venepuncture New patient medicals Immunisations (routine, childhood and travel) Womens health (cervical cytology, contraception, etc.)
Requesting pathology tests and processing the results, advising patients accordingly Travel medicine Chronic disease management awareness for: Diabetes Hypertension CHD Asthma Spirometry Understanding the importance of evidence-based practice Understand the requirement for PGDs and associated policy Ability to record accurate clinical notes Ability to work within own scope of practice and understanding when to refer to GPs Chaperone procedure Knowledge of health promotion strategies Desirable Broad knowledge of clinical governance Knowledge of public health issues in the local area Awareness of issues within the wider health arena Qualifications Essential Registered Nurse (Nursing and Midwifery Council) Desirable Mentor or teaching qualification Post graduate diploma or degree (Chronic Disease Management) Family planning qualification Experience Essential Experience of working in a primary care environment Experience of working autonomously Experience of working as a practice nurse or community nurse Experience of infection prevention and control measures Desirable Experience of managing elderly and housebound patients Experience of quality initiatives, , benchmarking Other requirements Essential Flexibility to work outside of core office hours Disclosure Barring Service (DBS) check Occupational Health clearance Meet the requirements and produce evidence for nurse revalidation Evidence of continuing professional development (CPD) commensurate with the role of a Practice Nurse
