Main duties of the job The post holder will deliver General Practice Nursing Services to the practice population where focusing on evidence-based practicefor long-term condition along with preventative condition management.
Candidate will be expected to provide the high-quality care for chronic disease management, cervical smear, Immunization, contraception, and health reviews.Candidate will be expected to work independently or with general practitioners, adhering to the NMC Code ofConduct and agreed practice standards and protocols.
The individual will alsoprovide nursing services/treatments to patients.Candidate will be expected to help lead and manage the local schemes such as primary care standards and data entry for the more complex vaccination and histories from outside the UK.

Job description Job responsibilities The main duties and responsibilities would include:1.
Provide nursing treatmentsto patients in participation with the general practitioners or independently toagreed protocols.2.
Provide general andspecific health screening to the practice population (within agreed protocols)with referral to general practitioners as necessary.3.
Organise undertake childhealth / travel immunisation clinics.4.
Order and maintenance ofvaccine stock and stock rotation.5.
Lead for Fridgemaintenance and Cold chain.
Lead for Infection Control.6.
Responsibility for calland recall and routine / proactive monitoring of patients on Chronic DiseaseRegisters (QOF).7.
Advice patients requestingtravel vaccinations organise and undertake vaccination programme whereapplicable.8.
Lead and manage Influenzacampaign.9.
Lead on diabetesmanagement if / when qualified to do so.10.
Advise on and administercontraception and depo injections.11.
Coordinate Practice auditas required and lead on infection control management and admin.12.
Advise patients on generalhealth care and minor ailments with referral to general practitioner asnecessary.13.
Participate in ongoingpractice projects such as secondary prevention of heart disease, PCQRRS, GMStandards and PCN Incentives such as IIF.14.
Maintain supplies andequipment used.
ensure stock control and room audits.15.
Maintain accurate patientrecords and statistics.16.
Chaperone when required byGP.17.
Take part in ProtectedLearning Time sessions with other practice team members and attend monthlytraining sessions.18.
Attendance at Fire,Infection Control and Resuscitation sessions annually plus all other requiredtraining and development.19.
Refer to other members ofthe multi-professional team as required.EDUCATIONAND TRAINING1.
Nursing Degree orRegistered Nurse degree apprenticeship2.
Asthma3.
COPD Training as required,including Spirometry.4.

Prescribing Course(Optional)9.
All routine training suchas Information Governance / Safeguarding / Infection Control as required by thePractice.COMMUNICATION1.
A regular daily and / orweekly meetings with GPs on clinical issues.2.
Regular communication withPractice Manager and Practice team.3.
Accurate record keeping.4.
Daily review of NHS Emailand internal email system.5.
Daily review of internaland external tasks such as EMIS and Docman.6.
Regular review of GPContract, revalidation, top up indemnity and NMC membership - requirement onthe candidate to ensure this is all up to date.KEY RESPONSIBILITIES1.To provide assessment, screening and treatment services whichare relevant to patients and their families, at the Practice, or in the patientshome.
Manage and Lead on QOF / IIF / PCQRRS & other projects as required bythe Practice.2.To participate in the administrative and professionalresponsibilities of the practice team.3.Participate in data entry as relevant to the role andrequirements.4.To assist in the formulation of practice philosophy, strategyand policy, and regular update of relevant practice protocols.5.To assess and monitor patients with chronic disease andrecommend referral to GP for change in treatment.6.To participate in research within the Practice.7.To liaise with staff in hospitals and community, social servicesstaff and voluntary organisations, as required to provide an adequate serviceto the patients.8.To maintain professional development by attending courses andstudy days as arranged with the Practice Manager/GP.9.To co-operate with the University and School of nursing ifrequested in allocation of student nurses to the practice.10.
To take fullresponsibility for all areas of any prescribing (if within remit of role andcapabilities) and act on any requests or audits from the Medicines ManagementTeam.11.
Ensure Safeguarding dutiesand responsibilities are undertaken to the fullest extent.12.
Ensure Infection Controlmeasures are adequate and followed to the upmost extent - Lead for this areawithin the Practice.13.
Ensure continuousprofessional development and training.
Person Specification Qualifications Essential Registered General Nurse Full NMC registration Qualifications/Certificates in cervical screening, immunisation administration, relevant to chronic disease management, and other areas relevant to post Recognised qualification to diploma level or above Experience of working in a GP practice Working via EMIS A demonstrable commitment to professional development Desirable Previous experience of Care Quality Commission evidence table to achieve nursing-related indicators to a high standards Stable career history showing clear commitment and progression Independent prescribing qualification.
Experience Essential Experience of working in accordance with the NMC Code of Professional Conduct and The Scope of Professional Practice Experience of EMIS clinical system Experience of dealing with the public/patients.
Experience of management of patients with long term conditions.
Ability to record accurate clinical notes
