Job summary WE ARE HIRING FOR ONE POSITION 3 DAYS A WEEK TUESDAY, WEDNESDAY AND FRIDAY TO ONE HOUR FOR LUNCH UNPAID TUESDAY AND FRIDAY WILL BE BASED AT SOUTH MILFORD SURGERY AND WEDNESDAYS WILL BE AT BRAMHAM MEDICAL CENTRE WHICH IS PART OF WETHERBY SURGERY.
LOCATIONS AND DAYS MAY DIFFER IF REQUIREDTo be responsible for the implementation of processes for the effectivemanagement of patients with long-term conditions using evidence-based practiceincluding care for elderly and housebound patients.Working as a senior member of the practice multidisciplinary team, the post holderwill ensure nursing services are delivered effectively to the entitled patientpopulation.The Practice Nurse will be responsible for a number of clinical areas such asinfection prevention and control, chronic disease management, health promotionand screening, immunisation of children/adults, contraception and wound care aswell as actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.Wetherby surgery & South Milford Surgery must ensure that the postholder has access to appropriate clinical supervision and an appropriate namedindividual in the organisation to provide general advice and support on a day-to-day basis.

This will be dependent uponfactors such as workload and staffing levels:a.
To act support the Infection Prevention Control (IPC) lead and to work to theIPC Policy providing audits, training, support and evidence of compliance tothe practice managerb.
To develop, implement and embed an effective care managementprogramme for the frail and housebound patients registered at the practicec.
To assess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practiced.
To develop, implement and embed health promotion and wellbeingprogrammese.
To implement and evaluate individual treatment plans for chronic diseasepatients that promote health and wellbeingf.
To identify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients healthg.
As required, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSFh.
To review patient medications to enhance compliancei.
To co-ordinate the provision of travel medicine servicesj.
To request pathology services and process pathology results as requiredk.
To provide chronic disease clinics, delivering patient care as necessary,referring patients to secondary/specialist care as requiredl.
To maintain accurate clinical records in conjunction with extant legislationm.
To ensure read codes are used effectively by all members of the nursingteamn.
To ensure the appropriate maintenance and use of chronic diseaseregisterso.
To provide other clinics for patients, including cervical screening &contraceptive reviews, also Diabetes and Respiratory clinicsp.
To ensure compliance with the practice chaperone policyq.
To assist GPs with minor surgery when requiredr.
To prioritise health issues and intervene appropriately s. To support the team in dealing with clinical emergencies t. To recognise, assess and refer patients presenting with mental health needs u.
To implement vaccination programmes for adults and children v. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice) w. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) y.
To support the clinical team with all safeguarding matters, in accordance with local and national policies z.

Develop and review audit protocols and process for the practice, effectivelyutilising the audit cycleb.
Develop, review and implement nursing protocols in conjunction with thepartnersc.
Participate in local initiatives to enhance service delivery and patient cared.
Support and participate in shared learning within the practice in order toimprove patient caree.
Continually review clinical practices, responding to national policies andinitiatives where appropriatef.
Participate in the review of significant and near-miss events applying astructured approach, , root cause analysis (RCA)g.
