Job summaryWe are looking for an experienced Practice Nurse to join our nursing team.We are a multidisciplinary Practice that includes GPs, ClinicalPharmacists, Practice Nursing, Health Care Assistants, and Physiotherapists.
Main duties of the job To be fully proficient in Chronic disease management, vaccinations andimmunisations, cervical screening, contraceptive reviews, wound care includingcompression, as well as other general treatment room duties.To provide support to junior members of the nursing team includinghealthcare assistants and will direct their work to ensure that the Practicesprovide high quality care.You will oversee infection control standards and cold chain compliancy.
About us To work within NMC Code of Conduct and to agreed Practice standards andprotocolsOversee recall systems for chronic diseases, cervical screening,immunisations, and other key areas as relevant to Practice NursingDelivery effective implementation of management of patients with ChronicDiseases as detailed by the Quality and Outcomes Framework, local qualityschemes and national quality schemesSupport the implementation of evidence-based guidelines in conjunctionwith the wider teamEncourage the development of integrated services within Primary Care toinclude integrated practice-based teams and multidisciplinary workingOversee infection control and cold chain systems to ensure compliancyEnsure that all nurse-related Care Quality Commission evidence tableindicators are compliant, demonstrating evidence, review, and audit cycle Job description Job responsibilities Participate in an annual appraisalActively participate in clinical and practice meetingsDevelops clinical protocols for the surgery and the team.Clinical dutiesProvide Chronic disease management for Diabetes, COPD, Asthma, andCHD/CVDPerform cervical screening and promote breast awarenessAdminister adult and paediatric injectionsProvide adult and child immunisations ensuring full achievement oftargetsEnsure safe storage, rotation, and disposal of vaccines and drugsUndertake venepuncture and electrocardiographyUndertake wound care/ management / dressing techniquesUndertake leg ulcer management, dopplers and compression bandagingProvide advice on lifestyle issues, assisting and supporting patients inchanging their unhealthy behaviours and refer appropriatelyParticipate in the smoking cessation serviceDeliver travel health care, including vaccinations and advice forpatientsRecognise the signs of child abuse, family violence, vulnerable adults,substance abuse, addictive behaviours, suicidal tendencies, and be aware oflocal, referral proceduresWork with safeguarding teams where relevantEnsure that current evidence-based approaches to patient care arestandard practiceApply infection control measures when collecting and handling oflaboratory specimens, disposing of waste materials, dealing with blood and bodyfluids spillages and report and treat sharps injuriesUndertake first aid and management of emergencies, burns,haemorrhages, hypo/hyperglycaemic emergencies/asthma attacksProvides a comprehensive family planning serviceAudit outcomes of care against standards and initiate change as necessaryensuring follow-up audit cycleMaintain and update disease registers Person Specification Qualifications Essential Nursing degree or other relevant nursing qualification granting professional license to practice primary care nursing Qualified in chronic disease management for Diabetes, Asthma, COPD and CHD/CVD Wound dressing and leg compressions Cervical screening qualification Venepuncture and electrocardiography Desirable Previous experience of Care Quality Commission evidence table to achieve nursing-related indicators to a high standards Stable career history showing clear commitment and progression Independent prescribing qualification Experience Essential Positive attitude Ability to multitask Strong planning and organisational skills Excellent communication skills Highly motivate Excellent telephone manner Excellent negotiation skills Demonstrable problem-solving experience Highly experienced Practice Nurse Experienced in delivering chronic disease management Experienced in delivering cervical screening Experienced in vaccinations, injections, and immunisations Experienced in wound care management, dressing techniques, leg ulcer management and dopplers.
Experienced in overseeing infection control systems to achieve compliancy.
Experienced in maintaining cold chain compliancy Desirable Experience of working in a GP practice Working via System One
