Supported by the GPs within the practice, they will recogniseand work within their own competence and professional code of conduct asregulated by the NMC.
The post holder will make an assessment of the patientsneeds and act upon their assessment.
Produce accurate, contemporaneous and complete records of patientconsultations, consistent with legislation, policies and procedures.
They willact collaboratively with the general practice team to meet the needs of patientsby providing quality evidence based care.
They will support the delivery ofpolicy and procedures and will be responsible for maintaining a personaldevelopment plan.The post holder willhave aresponsibility to support the practice in achieving its targets for, chronicdisease management, public health initiatives, LES, DES and QOF.
It isessential that you ensure patients health checks and records are up to date atall times.
Where patients are requiring periodic checks for chronicdisease you are required to carry out those checks and to ensure that any otherchecks which are due within a short timescale are also carried out so as toimprove lean working within practice and improve patient experience, byreducing number of times they have to come to the surgery Person Specification Qualifications Essential Qualified registered nurse with experience in the following: Diabetes COPD Asthma Immunisations Contraception Experience Essential Experience working in Primary Care
