Job summary OrchardSurgery has an exciting opportunity for an experienced nurse to join ourfriendly practice.
A fantastic multidisciplinary team will support you,comprising of GPs, paramedics practitioners, clinical pharmacists, practicenurses, HCAs, care coordinators, a health coach and we arecurrently recruiting a social prescriber.
Allstaff have the opportunity to extend their roles and develop new skills.
Thereare opportunities for leadership and specialisation.
Some of our carenavigators are trained to do ECGs and urine dips, freeing up our nurses toundertake more specialised care.
We are currently looking at the role of groupconsultation for diabetes and other LTCs having successfully implementedmenopause group nurses have regular meetings as a nursing team, fortnightly clinician meetingsin practice and collaborative nurse led meetings in the PCN.
You will also haveaccess to regular mentoring and clinical supervision.OrchardSurgery is part of the new Park and Orchard PCN in Horsham.
We workcollaboratively together and with our local community in partnership.
Ourcurrent focus is on improving proactive care for our frail elderly houseboundpatients and access to womens is a strong focus on Staff Wellbeing in the practice.
We close the practicefront doors for 1 hour at lunchtime to allow staff to take a break, have lunchtogether or go for a walk.
We also offer training sessions at lunchtimes.

Job description Job responsibilities Key Responsibilities:Identify,diagnose and manage treatment plans for patients at risk of developing along-term condition, as appropriateWorkwith other health care professionals to diagnose, monitor, manage & treatlong-term conditions including non-drug-based treatment methods using amanagement plan, and in line with national and local policies and practiceneedsInfectioncontrol policies, training and implementationReviewmedication for therapeutic effectiveness, appropriate to patient needs and inaccordance with evidence-based practice, national and practice protocols with GP supportPrioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycareSupportpatients to adopt health promotion strategies that promote healthy living, andapply principles of self-care, utilising community assets such as the HorshamWellbeing HubDemonstratesensitive communication styles to ensure patients are fully informed andconsent to treatmentParticipatein planning and implementing changes within the area of care and responsibilityJob Responsibilities:Provideassessment, screening and treatment services and health education adviceParticipatein organising and co-ordinating the provision of nursing services for the practiceto include the delegation of duties to other practice team members asappropriateProvidenursing treatments to patients in participation with general practitioners orindependently to agreed protocolsProvidegeneral and specific health screenings to the practice patients- within agreedprotocols with referral to general practitioners as necessaryAdvisepatients on general health care and minor ailments with referral to GPs asnecessaryDelivervaccinations and immunisations as requiredMeetthe needs of patients presenting for wound careSuppliesand equipment treatment rooms and other areasEnsurethe maintenance of equipment and stock levels relating to patient carePathologicalspecimens and investigatory proceduresUndertake to support the administrative team inthe collection, storage and dispatch of pathological specimens including swabs,smears etc..Administrative and professional responsibilitiesParticipatein the administrative and professional responsibilities of the Practice teamEnsureaccurate notes of all consultations and treatments are recorded in the patients notesand on the clinical computer system as appropriateEnsureaccurate completion of all necessary documentation associated with patienthealth care and registration with the PracticeEnsurecollection and maintenance of statistical information required for regular andad hoc reports, audits and research programmesAttendand participate in practice clinical meetings on a regular basisAssistin the formulation of practice philosophy, strategy and policy and developappropriate protocolsResearch projects and trainingCo-operateand participate as required in any research projects within the PracticeSupportstudent/staff education and training, in particular student nurses, HCAs andthose on work experience or inductionLiaisonMaintaineffective liaison with other agencies and staff concerned with patient care andwith all other disciplines within the Practice, with appropriate regard toconfidentialityProfessional/Personal developmentMaintaincontinued education by attendance at courses and study days as deemed useful ornecessary for professional or practice development, and participate in anytraining programme implemented by the practice as part of this employmentParticipationin an annual individual appraisal, including taking responsibility formaintaining a record of own personal and/or professional developmentTakingresponsibility for own development, learning and performance and demonstratingskills and activities to others who are undertaking similar work includingparticipation in clinical supervision and acting as a positive role modelSupportthe development of others in order to maximise potentialConfidentiality:Havean awareness that patients entrust us in the course of seeking treatment, withsensitive information in relation to their health and other matters.
