Job summary Oaklands Health Centre is a dynamic surgery that has developed significantly over the few years.
We have played a key role in the Covid response within our area and have a strong and well established clinical workforce.Our clinical team consists of 5 Advanced Nurse Practitioners, 1 Paramedic, 2 Practice Nurses, 2 Specialist Nurses and 3 Health Care Assistants.
We are looking to expand our team by recruiting an additional Practice Nurse to cover allaspects of primary care nursing including cervical cytology, immunisations, wound care, LTC management and QOF.
Main duties of the job Core duties will focus on providing excellent patient centered care whilst supporting the practice in achieving quality indicators and requirements of the GMS contract and any relevant National Service Frameworks.
This includes -Experience in Chronic Disease Management / willingness to undertake trainingUndertake opportunistic screening when clinically required.Perform bloods and ECGsUndertake wound care managementLeg ulcer managementCarry out ear examination and syringing, following appropriate training and assessment.Perform cervical screening and promote breast awareness.Administer adult and paediatric injections under an individualised prescription or PGDs, ensuring safe storage, rotation and disposal of vaccines and drugs.Provide an adult and childhood immunisation service working to PGDs - following appropriate training if required.Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.Deliver travel health care, including vaccinations and advice for patients - following appropriate training if required.Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies and be aware of local, referral procedures.Ensure that current evidence based approaches to patient are standard practice About us Oaklands Health Centre is a dynamic surgery that has developed significantly over the last few years.
