Job summary We are looking for a practice nurse to join our friendlyteam working in the town of New Mills in the beautiful High Peak, close toStockport.
We are a fairly smallpractice, with around 4600 patients currently registered with us.

We are looking for someone whowill enjoy working as part of our multidisciplinary team and is also confidentto work independently within their professional capabilities.
Webelieve that this role would best suit someone who is already experienced inworking independently as a practice nurse, but we would consider applicants whohave other relevant experience and a desire to work within GeneralPractice.
We are looking for someone towork part time, between 18 and 24 hours/week.
Main duties of the job The successful candidate will be responsible for thedelivery of the full range of practice nursing services, delivering care withintheir scope of practice to the entitled patient population.
The practice nursewill be responsible for a number of clinical areas such as infection preventionand control, health promotion, chronic disease management, health prevention,as well as supporting the management team in the reviewing of clinical policyand procedure.
About us Previously mentioned, we currently have 4600 registered patients.
Ourpractice area covers New Mills, Hayfield, and the surrounding area, extendingpart way into Disley at one end and Whaley Bridge at the other.
We use EMIS as our clinical system.This is a beautiful area in which to work, and we arefortunate to work from a large purpose built premises in the centre of NewMills.
We are a part of the High Peak and Buxton PCN, which is madeup of 8 practices covering the area from New Mills to Hartington.
We are active members of this PCN, and weenjoy good working relationships with all our neighbouring practices.Our clinical team currently comprises 2 GP partners, 1salaried GP, 1 practice nurse, 2 HCAs and 1 clinical pharmacist, andare well supported by the widercommunity and PCN multi-disciplinary team.We are a training practice who welcome GP specialisttrainees as well as medical students.We are looking to recruit an ANP as a new role alongsidethis practice nurse appointment, which we hope will provide additional clinicalexpertise to our team and provide additional support to our nursing team.
Our clinical pharmacist also carries out someof the chronic disease reviews.We have a weekly clinical lunchtime meeting which gives theteam an opportunity to discuss challenging cases or scenarios and discuss any new developments within the practice.
We close once a month for full team training.
Someof these sessions are joint with our PCN.
Job description Job responsibilities Responsibilities and duties will include, but may not belimited to the following Practice independently in accordance with NMCcode of conduct and in line with practice and national guidelines.Maintain accurate documentation of care andconsultations.Ensure continuation of own personal andprofessional development, facilitated by support from practice.Wound care in line with local guidelines includes suture removal and simple wound care.More complex wounds are managed by our local wound care clinic.Administration of child and adult immunisationsin line with national and local guidance.Ensure safe storage, rotation and disposal ofvaccines and drugs and adequate stock levels are maintained.Responsibility for reviewing and maintainingclinical aspects of the infection control policy within the practice.Mentoring/clinical support to HCAs.Cervical cytology.Phlebotomy and ECGs where needed.Health promotion/prevention where appropriate.CVD risk assessment eg QRISK calculations.Chronic disease managementAble to discuss new diagnoses of conditions suchas diabetes.To carry out reviews on patients with the fullrange of long term conditions eg hypertension, CVD, asthma, COPD, diabetes,in line with recognised national guidelines and in accordance with QOFstandards.Participation in audits/quality improvementactivities within the practice, alongside the wider team.Participation in weekly practice clinicalmeetings and monthly training sessions.
Person Specification Qualifications Essential Qualified nurse NMC registered Clean driving license Desirable Experience of working in General Practice as a practice nurse.
Cervical cytology and immunisation training/qualifications.
Experience of or any specific qualifications in chronic disease management.
Mentor/teaching qualification or experience.
Recognised Infection Control Qualification.
