Job summary We are looking for full or part time Practice Nurse to join our team at Swanscombe Health Centre and branch practices.Based at Swanscombe Health Centre, the role of the Practice nurse will be to work within our nursing team, to offer support and treatment for our patients, focusing particularly on health improvement.
You should have experience of working in a similar role, with relevant qualifications, NMC registration, and have excellent organisational skills.
Main duties of the job Duties will include (but not exhaustive); Baby/Childhood immunisations Injections Wound care Cervical Screening Pill Checks/contraceptive clinics Long term condition checks/support.
General PN duties About us We have a team of over 100 staff throughout 7 sites, including GPs, Paramedic Practitioner, Clinical Pharmacists, Physicians Associates, a Physiotherapist, Practice Nurses, Healthcare Assistants, Social Prescribers and the administrative support workers.

Job description Job responsibilities The main duties and responsibilitiesare as follows.1.Meetingpatients nursing care needs as identified, or where care has been transferredto them by a GP.
This will usually be insurgery but occasional home visits will also be required.2.Initiatingand maintaining health screening and promotion clinics for identified areas ofthe practice population, recognising where modification of systems or referralis necessary.3.Managingthe nursing care of patients, employing personal recall systems suitable to thepractice needs.
