What You'll Do - Provide high-quality patient care within a collaborative and supportive team environment.
- Manage chronic disease cases and offer specialised support in areas like diabetes care and long-term conditions.
- Conduct first-contact assessments and provide telephone triage.
What We're Looking For - Qualified Practice Nurse with a minimum of two years' experience in a primary care setting.
- Proven background in chronic disease management.
- Exceptional teamwork, communication, and interpersonal skills.
- Experience with long-term conditions is desirable.
