We have an exciting opportunity for an experienced Practice Nurse in Cherry Hinton & Brookfields Medical Centre Main duties of the job Main duties of the jobWe are seeking an experienced and enthusiastic Primary Care Nurse with a strong understanding of quality assurance.The postholder will actively engage in practice / patient engagement and will play a vital role in providing high-quality care to our patients.Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management, and preventative nursing interventions to all patients.Assist the practice and its staff with compliance of infection prevention and control policies and proceduresAssure clinical safety and drive excellent patient outcomesSupport the practice manager and clinical lead in service deliveryWork collaboratively with the whole practice team to meet the needs of patients, supporting the delivery of policy and procedures.
About us Overview of your organisationWe are Cambs GP Network - a Federation of Practices across Cambridge and surrounding villages, with a social enterprise ethos - wholly owned by our local member practices.
We hold the contract to deliver services at Cherry Hinton & Brookfields Medical Centre.Cherry Hinton & Brookfields Medical Centre has nearly 10,000 patients - cared for by a supportive, experienced team and practice manager.
The clinical team includes a full cohort of GPs, ANP and nurses who have been with the practice for many years.
We have an excellent team of clinical coders, prescribing ANP, PCN and an in-house pharmacy team to support with admin, results and med reviews in order to reduce GP workload.
The practice is located in South Cambridge, an easy cycle or drive to central Cambridge amenities and conveniently located on the outskirts for anyone travelling from outside the city.This is a traditional practice supporting old-fashioned face-to-face high-quality consulting, that has the support of a bigger network in the form of CGPN so you can focus on patients, not firefighting and be sure of the transparent and stable running of the practice.Full or Part-time hours considered.Monthly clinical closure afternoon for training and team building.SystmOne practice with full Ardens templates.NHS Pension.
Job description Job responsibilities Role SummaryCambs GP Network has an exciting opportunity for a Practice Nurse to join our committed, hard-working and multi-disciplinary team of clinicians at Cherry Hinton & Brookfields Medical Centre.
The post holder is responsible for ensuring delivery of safe and effective nursing care to the whole practice population and accountable for nursing service delivery.
The post holder will be an enthusiastic Primary Care nurse with a strong understanding of quality assurance.This role will additionally allow the post holder to work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures.Job Responsibilities: Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.
To identify, manage and support Patients at risk of developing long term conditions, preventing adverse effects to patients health Conduct regular reviews and assessments of patients with long-term conditions, adjusting treatment plans and providing education to optimise self-management.
Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services, as necessary.
Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients.
Administer vaccinations and immunisations, including travel (depending on individual competency), according to national guidelines and practice protocols.
Support and manage health needs of women presenting with contraception and cervical screening.
Complex wound management Venepuncture and request Pathology services as required Support the team in dealing with acute clinical requests and clinical emergencies To act as chaperone for patients as needed To support the clinical team with all safeguarding matters in accordance with local and national policies The role of the Practice Nurse is an evolving one and it is anticipated that the post holder will be responsible and reasonably flexible in the interpretation of the above job description.
Person Specification Qualifications Essential Registered Nurse with valid NMC registration Clinical Knowledge & Skills Essential Wound Care ECGs Venepuncture Immunisations (routine, childhood & travel) Women's Health (cytology, contraception) Understand the requirements for PGDs and associated policy Desirable Travel Medicine Diabetes Hypertension CHD Respiratory (Asthma & COPD) Personal Qualities Essential Polite and Confident Motivated and forward thinker Effectively able to communicate and understand the needs of the patient Commitment to ongoing professional development Experience Essential Experience of working in a primary care environment Experience of working as a practice nurse or community nurse Desirable Experience of chronic disease management Skills Essential Excellent Communication skills Strong IT Skills System One User Effective Time Management Ability to work as a team member and autonomously Problem Solving & Analytical skills Ability to follow clinical policy and procedure Desirable Experience with audit
