We hold the contract to deliver services at Cherry Hinton & Brookfields Medical Centre.Cherry Hinton & Brookfields Medical Centre has nearly 10,000 patients - cared for by a supportive, experienced team and practice manager.
The clinical team includes a full cohort of GPs, ANP and nurses who have been with the practice for many years.
We have an excellent team of clinical coders, prescribing ANP, PCN and an in-house pharmacy team to support with admin, results and med reviews in order to reduce GP workload.

The post holder is responsible for ensuring delivery of safe and effective nursing care to the whole practice population and accountable for nursing service delivery.
The post holder will be an enthusiastic Primary Care nurse with a strong understanding of quality assurance.This role will additionally allow the post holder to work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures.Job Responsibilities: Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.
To identify, manage and support Patients at risk of developing long term conditions, preventing adverse effects to patients health Conduct regular reviews and assessments of patients with long-term conditions, adjusting treatment plans and providing education to optimise self-management.
Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services, as necessary.
Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients.
Administer vaccinations and immunisations, including travel (depending on individual competency), according to national guidelines and practice protocols.
Support and manage health needs of women presenting with contraception and cervical screening.
