*PLEASE NOTE THIS IS NOT AGENDA FOR CHANGE* Main duties of the job You will work collaboratively as part of our multidisciplinary sector team of clinicians to meet the needs of our patients; delivering the highest standards of patient care.
You will undertake a diverse range of nursing assessments and provide the appropriate care / treatment according to patient needs, striving to improve standards of care and enhancing nursing services within the practice.
Depending on experience, Long Term Chronic Disease management will also form a large part of the role as well as the mentoring of our Health Care Assistants, Assistant Practitioners, Nurse Associate Students and RGN Student Nurses.
Knowledge of the GMS contract The Quality and Outcomes Framework, will be valuable to the post.
