hours - RPC Brooklyn, Heanor - must be experienced We have a team of 200 people dedicated to our patients, working across 3 sectors covering Chesterfield, Clay Cross and Heanor.
Our Practice Nurses are essential to ensuring that we always provide an outstanding level of nursing care to our patients.

*PLEASE NOTE THIS IS NOT AGENDA FOR CHANGE* Main duties of the job You will work collaboratively as part of our multidisciplinary sector team of clinicians to meet the needs of our patients; delivering the highest standards of patient care.
You will undertake a diverse range of nursing assessments and provide the appropriate care / treatment according to patient needs, striving to improve standards of care and enhancing nursing services within the practice.
Depending on experience, Long Term Chronic Disease management will also form a large part of the role as well as the mentoring of our Health Care Assistants, Assistant Practitioners, Nurse Associate Students and RGN Student Nurses.
Knowledge of the GMS contract The Quality and Outcomes Framework, will be valuable to the post.

Person Specification Qualifications Essential Registered General Nurse Registered qualification level, diploma or above Evidence of a commitment to continued professional development Desirable Further accredited qualification in any chronic disease Mentorship qualification and/or sign off mentor Experience Essential Experience of working in accordance with the NMC Code of Professional Conduct Experience of dealing with the public in a respectful and professional manner Experience of a multi-skilled approach to Chronic Disease Management Understanding the need for confidentiality at all times Desirable Practice qualification or experience of working in primary care Evidence of working in a complex organisation, demonstrates sound knowledge or awareness of the role of a nurse in primary care Experience of TPP SystmOne clinical computer system or equivalent Evidence of involvement in research, training and development Personal Attributes Essential Able to demonstrate enthusiasm towards developing personal nursing skills, and be excited to nurture and mentor other members of the team Excellent time management skills and professional appearance and approach Able to work on own initiative without direct supervision and determine own workload priorities within the boundaries of the role Caring and compassionate individual Well presented with excellent time management skills and professional appearance and approach Able to work on own initiative without the direct supervision and determine own workload priorities within the boundaries of the role Able to use own judgement, resourcefulness and commonsense but able to listen to others and encourage feedback in approachable, positive manner Recognise own limitations towards learning and development, identifying gaps in training and knowledge and provides solutions to achieve these goals for progression Demonstrate an ability to work as part of an integrated multi-skilled team, respecting the importance of each and every member Easily adapt to alterations in performance, pace and daily pressure to meet the patients' needs within an ever-changing environment Flexibility of working days and hours including extended hours provision/able to travel to different locations at short notice Hold a full clean driving license Desirable Leadership and mentoring skills
