Job summary The Suffolk and North East Essex (SNEE) Integrated CareBoard (ICB) is seeking to recruit a Data Analyst to deliver analytical work inpopulation health management (PHM).
PHM is an approach to healthcare that useslinked data to design and implement proactive models of care which will improvehealth and wellbeing today as well as in the future.As a key member of the PHM analytics team, the post holderwill play a role in developing and enabling PHM insights from data andanalysis.
They will work with stakeholders across organisations to provide dataanalysis and communicate insights.
They will also play a supporting role inenabling stakeholders to access, interrogate and interpret insights from theICSs analytics environment and reporting platform.The wider SNEE Integrated Care System (ICS) has a diverseand talented analytical community supported by modern data and analyticstechnology and engaged stakeholders.
The data professional community comestogether as the SNEE Intelligence Function, which is working in novel andcollaborative ways to create new, impactful insights from data and to establisha positive environment for our data professional staff to develop.The successful candidate will have excellent knowledge andskills in analysing and interpreting complex data.
They will have stronganalytical communication skills, and will demonstrate a desire to improveperformance and services.
Main duties of the job Togenerate insights from data by querying, analysing and modelling datasets usingan array of analytical techniques.
Providerelevant and timely information to inform patient care.
Assistingin producing regular and ad hoc analysis for stakeholders.
Developspecialist analytical skills.
Assistin the development, design and maintenance of data visualisation reportingtools.
Assistin provision of advice and support/guidance on specialist analysis.
Implementationof policies and processes for own area, especially those relating toinformation governance and protection of patient level data.

PopulationHealth Management (PHM) aims to improve physical and mental health outcomes,promote wellbeing and reduce health inequalities across an entire population.It supports population health by helping us understand our current health andcare needs and predict what local people will need in the future.
The aim is touse data to design and implement proactive models of care which will improvehealth and wellbeing today as well as in the future.TheSNEE Integrated Care System (ICS) is now implementing its PHM Strategy and enhancingits analytical capabilities through the development of a SNEE ICS IntelligenceFunction.
This is an analytical collaborative, with significant investment indata science and advanced analytics capabilities, ensuring SNEE ICS is wellserved by a sophisticated and high performing analytical function.
SNEE ICS hasenhanced its infrastructure and intelligence capabilities by developing alinked, longitudinal dataset.
This brings together care records across primarycare, secondary care, and adult social care to enable analysts to identifycohorts of the population with risk factors or long-term health needs.
Inaddition, the SNEE ICB has established a cloud-based integrated data andanalytics environment with modern analytical tools to enable the exploitationof the linked data capabilities.
Thepost holder will play a supporting analytical role in developing and enablingPHM insights from data and analysis.
Their analysis will include working withdata from across primary, secondary and care sectors, as well as data on widerdeterminants of health.
They will work with senior analytical managers toprovide data management and data analysis to support segmentation modelling,risk stratification, analysis of full patient pathways, and identification andmonitoring of health inequalities.
They will also play a supporting role inenabling stakeholders to access, interrogate and interpret insights from theICSs population health insights platform.
They will work with stakeholdersacross health and care as part of a virtual network of analysts in the SNEEIntelligence Function.
They will use linked data to tailor the provision ofcare to identify people who would benefit most from proactive intervention andto tailor services to best meet population need.
