An exciting opportunity has arisen for a registered Occupational Therapist/Social Worker/or Registered Mental Health Nurse or equivalent professional to play a pivotal role in delivering this service, as well as planning, evaluating, and contributing to its development.
We are looking for a professional who is driven, passionate and excited to be part of a strategically important service provision, in line with our programme of Community Mental Health Transformation which is aiming to transform and enhance the way we deliver our mental health services and aligned with the Trust's vision as 'One-Team'.
The service will employ an innovative model to provide out of area service delivery, gatekeeping for S117 after care applications, out of area placements/independent hospitals and individual funding requests for specialist complex mental health care for people with severe mental health issues.
The role will offer an opportunity to make a real difference to people's lives by working very closely with the mental health locality teams, ICBs, Local Authorities, Care Providers, and other key stakeholders.
The role will offer autonomy, variety, and significant development opportunities, both in leadership and clinical skills.
Part time hours will be considered between 32 - hours a week.
Main duties of the job To provide a safe and effective provision of Out of Area Placements (OAPs) services on a daily basis.
To ensure that Out of Area Placements deliver Safe, Effective, Caring, Responsive and Well-Led service with desired outcome as per care plan.
This will include delivering a service to enable service users to live healthy, fulfilling lives in a least restrictive manner as well as working across boundaries in a seamless manner to maximise a 'value for money' care provision to achieve the best possible outcomes for our service users.

Person Specification Education/Qualifications/Training Essential Degree in health/social care or equivalent qualification/experience.
Registered with UK professional body.
Experience of clinically supervising junior and unqualified staff and mentoring students, qualification attained in mentoring would be ideal but not essential.
Continuous Professional Development Essential Good level of IT skills.
Evidence of further training and professional development.
Previous Experience Essential Experience of reviewing independent hospitals and community placements to establish their clinical appropriateness, effectiveness and 'value for money' Experience of operating successfully within the Care Programme Approach.
Substantial experience of managing complex caseloads and applying a range of interventions Experience of working effectively in a multi-disciplinary team and multi-agency environment.
Knowledge, Skills & Abilities Essential Excellent knowledge and understanding of the health and social care systems and operating frameworks, clinical observations, and the Mental Health Act.
Knowledge and experiencing of managing situations concerning safeguarding, risk management, complaints, and investigations.
Sound negotiating skills.
Specialist knowledge acquired through training in a specialised discipline of healthcare, preferably mental health, although not essential.
Strong organisational skills and the ability to analyse situations and be able to make the right decisions.
Exceptional interpersonal, influencing, communication, observation and reporting skills.
Ability to engage and communication with a wide range of professionals, staff, and other service stakeholders.
Ability to work to timescales and comfortably cope in pressurised situations, applying practical problem-solving skills in everyday and complex situations.
Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances and to have the ability to stand back and use sound judgement on decisions or recommendations relating to out of area placements.
Competent IT skills and the ability to navigate around various systems and software packages (such as Rio, outlook, databases, MS office) Ability to work autonomously and effectively in a team, reprioritising work and that of others work to reflect changing needs.
Good decision making and judgement skills.
Manage difficult situations with service users that have complex mental health conditions and challenging behaviours.
Knowledge & Experience of spot purchasing and negotiation with Local Authority Procurement and Contracting.
Knowledge and experience of the S117 aftercare provisions, legislations and frameworks.
Additional Requirements Essential Must be able to work on a flexible basis to meet the needs of the service.
Reliable and resilient.
Ability to travel to multiple sites and locations, including out of area.
Valid UK Driving Licence and access to own vehicle to meet the demands of the work.
