Job summary The Worcester City communityphysiotherapy team are looking for a dedicated and passionate physiotherapistto join their friendly team.
Thesuccessful candidate will have the opportunity to work with a varied caseloadcovering the whole of Worcester city and St Johns.
The team consists ofphysiotherapists and physiotherapy assistants.We work closely with district nurses, occupational therapists, advancedclinical practitioners and alongside the GPs to support people living in theirown homes.Our working hours are typicallyMonday to Friday between the hours of however there is scope forflexibility and the opportunity to pick up additional shifts at the weekend.
This post is for hours per week.We are based at Warndon clinic,Cranham Drive however the whole team have laptops to allow home working asnecessary and when agreed with your line manager.We are a busy team, but have afriendly and supportive attitude.
Althoughthe successful candidate will work autonomously there is always a member of theteam willing to assist as needed, with advice over the phone or during a jointvisit.
You will be supported during theinduction phase to ensure you feel confident and have a clear understanding ofhow the team and wider MDT work to facilitate rehabilitation in the communitysetting.
Main duties of the job Delivering a highstandard of physiotherapy assessment and treatment to patients in their ownhomeProvide advice on therehabilitation management of patients to families, carers and other members ofthe MDTBe responsible formanaging your caseload, ensuring all patients have appropriate patient centredgoals set, rehabilitation plans in place and follow up reviews diarisedBe responsible for thetriage of new referrals on your given day as part of the team rotaAssess prioritypatients as part of the urgent community response initiative and admissionavoidance, as well as supporting hospital discharges including trauma andorthopaedic brace management and planning domiciliary interventions tofacilitate independent livingTreat a wide variety ofpresentations including falls, orthopaedics, neurological conditions,musculoskeletal and respiratory patientsMaintain accurateelectronic patient records in line with documentation guidelinesDelegate and superviseappropriate tasks and treatment plans to the physiotherapy assistantsTake part in thesupervision of physiotherapy assistants and students Ordering of patientequipment and ensuring the safe and appropriate use of thisCommunicating with thewider MDT including GPs regarding any concerns about your patientsTake an active role in teammeetings, clinical audit, and service and team development About us At Herefordshire and Worcestershire Health and Care NHS Trust,we are working together to deliver outstanding care.

Person Specification Experience Essential A minimum of 2 years post graduate experience including work in a variety of specialities Experience of supervision of junior staff or students.
Experience of delegation to qualified / unqualified staff Desirable Experience working in a community setting Experience of supervision of junior staff or students Experience of implementing change in clinical practice / audit Additional Criteria Essential Able to meet the travel requirements of the post Qualifications Essential Diploma / Degree in Physiotherapy HPC registration Evidence of relevant post Graduate) courses / CPD maintained in portfolio I understand that I will be asked to provide original certificates to verify my qualifications at point of offer Desirable Relevant validated clinical course Evidence of relevant post graduate courses/CPD maintained in portfolio Knowledge Essential A range of evidence- based Physiotherapy clinical skills and knowledge appropriate to the role Knowledge of rehabilitation concepts and services A good level of knowledge in a wide range of physical and mental health conditions Knowledge of IT skills Skills & Abilities Essential Ability to communicate effectively in English in both verbal and written formats Ability to maintain comprehensive accurate and up to date clinical records
