Locum Community Physiotherapist (Band 7) – Home First Service
Location: Epsom (SDH&C Home First Service)
Car Driver Essential
Hours: 37.5 hours per week with possiblity of working over 4 days (Monday to Friday initially, with potential to extend into weekends)
Contract Duration: Until 31/03/2025 (subject to change)
Are you a skilled and motivated Physiotherapist with experience in discharge planning, community care, and inpatient services?
This exciting opportunity offers a chance to join the Home First Service, where you'll make a real difference in patients' lives by supporting their recovery and independence.

Key Responsibilities:
Attend ward and board rounds, working closely with acute and community services to ensure seamless discharge processes.
Conduct follow-up Discharge to Assess (D2A) visits in the community, ensuring safe and effective patient care transitions.
Liaise with multidisciplinary teams to support patients with their rehabilitation journey both in inpatient settings and at home.
Contribute to improving patient outcomes and reducing hospital readmissions through your expertise in community-based physiotherapy.
What We're Looking For:
HCPC Registered Physiotherapist with significant experience in community and inpatient settings.
Proven ability to manage D2A pathways and deliver high-quality care in diverse environments.
Strong communication and liaison skills, with a commitment to multidisciplinary teamworking.
A valid UK driving license and access to your own vehicle (essential for the role).
