As a Physical Health Support Worker, you will provide a comprehensive physical health assessment for people diagnosed with complex mental health.
As part of your caseload, you will be supporting individuals to understand about their health particularly in managing their physical health needs, reviewing appointments to reduce the health inequality these individuals face.As a support worker you will work closely with the existing primary and secondary care teams, providing excellent opportunities for partnership working with a range of organizations.
