Job description Job responsibilities We are looking for a confidentPhysical Health Lead Nurse (RGN)who can inspire, support and oversee the delivery of the very best care.
Youll be working40 hours a week (Monday - Friday, 9am - 5pm)atCygnet Hospital Kidsgrove.
Cygnet Hospital Kidsgrove is our new hospital for women in Staffordshire.
The service will be opening in 2025.
There will be two wards at the hospital; a 20 bed Acute service for women and an 11 bed Highly Specialised Personality Disorder Rehabilitation service.
Crocus Ward will provide a safe and stabilising environment for women who are experiencing an acute episode of mental illness and require an emergency admission.
Burleigh Ward will provide specialised support for women with a personality disorder and other complex needs.
Your day-to-day Assess, plan, develop, implement and evaluate programmes to promote health and well being, and prevent adverse effects on health and well being.
Overseeing and evaluating individual care and treatment plans for patients with physical health conditions Identify and manage as appropriate, care and treatment plans for patients at risk of developing physical health conditions Regular monitoring and interventions for patients with long term conditions such as diabetes, asthma, hypertension, epilepsy etc Prioritise physical health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self care Provide information and advice on physical health medication regimes, side effects and interactions Assessment and management of patients with wounds Work closely with the visiting GP overseeing GP referrals, annual physical health check referrals, referrals to specialist services etc Working alongside the ward Drs with phlebotomy and ECG services making sure all are done accurately and timely.
Responsible for physical health induction and training for all staff Implement the hospital wide smoking cessation programme Oversee referrals to dieticians, opticians, dentists, hearing tests, chiropody and SALT Regularly check physical health supplies/equipment and order as required Why Cygnet?

The ability to maintain documentation, undertake audits & demonstrate integrity in your role A good understanding of the latest nursing practices A commitment to patient recovery in order to effectively monitor, manage & reduce risk Compassion, resilience & a dedication to empower & support service user independence Desirable: The ability to deputise in a Manager role An interest in leadership, research & assessment Cygnethas been providing a national network of high-quality specialist mental health services for more than 30 this role youll have the chance to make a real impact on patients, service users & their families.
