Job summary Thepost holder is a Pharmacy Technician andis an integral part of the general practice team, working within theirprofessional boundaries.Thisrole will work under the supervision of the Clinical Pharmacistto ensurethe safe, accurate and timely supply of prescribed medication to patients.Theywill provide technical and administrative support to the clinicians.ThePharmacy Technician will supportthe organisations clinical teams with queries regarding medication frompatients.The PharmacyTechnician will complete audits regarding patient safety in theprescribing of medication and present the information for sharing with theclinical team.
Main duties of the job The Pharmacy Technician is to assume thefollowing key responsibilities in delivering health services.a.Undertakepatient facing and patient supporting roles to ensure effective medicinesuse through shared decision-making conversations withpatientsb.Conductmedicine optimisation tasks including effective medicine administration, support medication reviewsand medicines reconciliation.c.Support,as determined by the organisation, medication reviews and medicines reconciliationfor new care home patients and synchronise medicines for patient transfersbetween care settings and linking with local community pharmacistsd.Providespecialist expertise, when competent, to address both the publichealthand social care needs of patients including lifestyle advice, service informationand help in tackling local health inequalitiese.Maintaina central role in the clinical aspects of shared care protocols and liaise withclinical pharmacists for more complex patientsf.Assistin the delivery of medicines optimisation and management incentiveschemesand patient safety auditsg.Supportthe implementation of national prescribing policies and guidance within GPpractices, care homes and other primary care settings.

JOB SUMMARY To provide specialistpharmaceutical technical services, professional advice and direct support toPractice GPs, Nurses, Pharmacist, HCAs and Reception/Administrative Staff To liaise withpatients, clinical and non-clinical staff, practice, hospital and communitypharmacists and patient's carers to improve the pharmaceutical care ofpatients.
To develop the role ofthe Practice Pharmacy Technician to maximise support to all Clinical andNon-clinical practice staff to improve patient care in relation to all aspectsof medicines and their impact To provide professionalspecialist medicines advice to GPs, Nurse Practitioners, Practice Nurses,Community Pharmacists, Community Nurses and other health professionals topromote the safe, effective and high quality, cost effective use of medicinesin all prescribing decisions for the benefit of the Practice's patients Person Specification Experience Essential Experience of working within a healthcare setting as Pharmacy Technician Desirable -Experience of working in Primary Care Qualifications Essential GPhC-approved Pharmacy technician course.
