We are thrilled to announce an opportunity to join our team as a Pharmacy Technician, following the success of having technicians allocated to our practice from the PCN.Job Summary:As a Pharmacy Technician in our General Practice, you will play a vital role in supporting the Medicine Management team and ensuring patients receive the best possible care.
This position combines patient-facing responsibilities with analytical and administrative tasks, contributing to the optimisation of medication use and the improvement of prescribing processes.
Main duties of the job Key Responsibilities: Medication Reconciliation:Review and reconcile medications from hospital discharge letters to ensure continuity of care.
Audits and Drug Monitoring:Conduct regular audits and monitor drug usage to ensure compliance with best practices and safety standards.
Repeat Prescription Management:Handle repeat prescription issues and queries, promoting efficient processes such as repeat dispensing and online ordering.
Medication Reviews:Assist in conducting medication reviews to optimise patient outcomes and minimise clinical risks.
Collaboration:Work closely with the Medicine Management team to implement practice improvements and prescribing initiatives.
Patient Interaction:Communicate effectively with patients to provide guidance on medication use and address any concerns or queries.
Professional Development:Engage in continuous learning and development opportunities, supported by regular access to a GP and participation in team meetings.

Job description Job responsibilities Job description for PharmacyTechnician- Stream general prescription requests and work thereception teams, so as to allow GPs and clinical pharmacists to review the moreclinically complex requests.- Assist patients with responses to medication queries- Provide training and support on the legal, safe and securehandling of medicines, including the implementation and increased use of theElectronic Prescription Service (EPS).- Promote Electronic Repeat Dispensing (eRD) and onlineordering as appropriate to patient and practice needs.- Implement efficient ordering and return processes andreduce medication wastage, liaising with patients as needed.- Support the implementation of national prescribingpolicies and guidance within GP practices, care homes and other primary caresettings.- Help to organise and run various systems for the deliveryof QOF and locally commissionedenhanced services.- Review and develop policies in line with CQC compliance.- Develop relationships with other pharmacy professionalsand members of the multi-disciplinary team including primary care, communitypharmacy, secondary care and mental health.- Contribute to the delivery of effective daily processesfor the transfer of care between hospitals and GPs including effectivesignposting, record updates and relate communication.- Through the use of audit and action planning, help totackle health inequalities.The above list of duties is not exhaustive and may besubject to change as deemed necessaryConfidentialityIn the course of seeking treatment, patients entrust uswith, or allow us to gather sensitiveinformation in relation to their health and other matters.They do so in confidence and havethe right to expect that staff will respect their privacyand act appropriately.In the performance of the duties outlined in this jobdescription, the postholder may have access to confide initial informationrelating to patients and their carers, practice staff and other healthcareworkers.They may also have access to information relating to thepractice as a business organisation.
