37 hours per week Your role: To work as part of a team to plan, review and provide support to young people 16-25, both preparing to leave care and supporting into adulthood having left care.
To have responsibility for a caseload of young people and ensuring their needs are met.
To facilitate safety and stability for young people while helping them achieve independence at their own pace.
You will work with partners, for example, in Housing, Health and education training and employment to provide a high-quality service to our Care Leavers and work to meet their needs holistically.
About you: You will have an understanding of the challenges facing Care Leavers and how best to mitigate the adverse life experiences many have faced.
You will be passionate about improving the outcomes for care leavers and be able to work in a challenging and fast paced environment.
You must be confident in report writing and be able to advocate on behalf of young people where necessary.
Importantly you will want to work with young people and help them have a successful transition into adulthood.
You will commit to your corporate parenting role to its full capacity and ensure that Care Leavers receive services/support as you would expect for your own children.
