You will be responsible for providing operational management support to the practices within our Primary Care Network in Milton Keynes, known as OneMK PCN.
Our PCN was launched on September 1st 2023, bringing together our three services across Milton Keynes: Whitehouse, Neath Hill and Brooklands surgeries.
Main duties of the job The PCN Transformation Manager will form an integral part of the PCN leadership working alongside the Clinical Director and the PCN members.The post holder will act as the main point of contact of expertise, coordinate the successful achievement of all PCN DES contractual requirements, support the translation of national and local policies into meaningful changes, and support collaborative working to achieve improved outcomes for the local population.They will also be accountable for Financial Management, to ensure financial management and reconciliation of financial funds and maintaining accurate financial records.

Job description Job responsibilities The PCN Transformation Manager will be responsible for providing strategic oversight, staying informed on current and future changes impacting the service.
This includes overseeing operational planning, designing, implementing, and reviewing new services, as well as managing these services on a day-to-day basis for the PCN.The role will involve overseeing the creation, management, and maintain systems and processes to ensure that the PCN achieves optimal performance and accountability.

Person Specification Qualifications Essential Possesses a degree-level education or substantial relevant experience Formal Management Qualification or equivalent significant experience.
Desirable Hold a relevant Management qualification Relevant Project Management qualification Experience Essential Minimum of 12 months experience in a senior role working with/in Primary Care or similar role with transferable skills Clear evidence of managing financial accounts procedures and budget control Experience of working on GP clinical systems Experience of developing and implementing common systems and processes across teams and across service boundaries Experience of facilitating and managing change Desirable Experience of digital project management systems Experience in managing records and data protection issues Skills and Knowledge Essential Strong communication skills are needed to engage with senior managers, clinicians, and patients/carers on complex service development and changes issues Able to independently plan, organize, and coordinate work effectively Proven ability to work effectively in a team and motivate others.
Desirable Knowledge of QOF, NHS long term plan and Enhanced Services Understand the payment structures within primary care Project Management experience including QI methodology
