Job summary We would like you to come and join our team as a Mental Health Nurse to provide support, advice, and signposting to patients with poor Mental Health.
You will be a registered Mental Health Nurse, with the NMC.We will provide a supportive learning environment, helping you to set out and revise specific educational goals and ensure you receive appropriate clinical supervision.
We have a well-established team of Mental Health Nurses supporting our five practices.
You will be supported and encouraged to attend CPD enabling you to grow and develop within the role.We recognise and value the importance of Mental Health Nurses as part of our wider clinical team which comprises a wide range of colleagues including Care Co-ordinators, Health and well-being coaches, Social Prescribing Link Workers, Advanced Practitioners, Occupational Therapists, Pharmacists and Pharmacy Technicians; alongside the wider team including the Practices, Community Pharmacy, the Voluntary and Community Sector, as well as statutory service providers including the ICB.
Main duties of the job Mental Health Nurses in primary care complement the role of the GP and other Health Professionals as integral members of the multidisciplinary team.
To make the best use of clinical skills within the practice in order to promote well-being and maximise health gain for patients.
Provide a safe, high quality and accessible treatment room service to the practice population.
The successful candidate with be working alongside our existing Mental Health Nurse Practitioner to provide a much needed and well-utilised service for our locality.The post will evolve as we develop our services.

Person Specification Experience Essential Detailed knowledge and appropriate understanding of assessing patients with acute or chronic mental health conditions, procedures and treatment plans.
This includes signposting and referrals to appropriate internal and external organisations.
Ability to participate in reflective practice and clinical supervision activities.
Relevant Mental Health clinical experience Experience of working in MDT settings Experience in documenting and record-keeping of clinical information Desirable Experience of working in Primary Care.
Previous use of SystmOne Other Essential Able to meet Conexus Healthcare standards of attendance.
