Job summary APEX Primary Care Network (PCN) is seeking to recruit an additional healthcare assistant with an interest in frailty to join our growing EHCH and Frailty team combined of our Frailty Practitioner, Clinical Care co-ordinator, Senior Healthcare Support Worker and our EHCH care co-ordinator.This is an exciting and unique opportunity to be at the forefront of the delivery of our new Frailty service and implementation of the PCN DES 'Enhanced Health in Care Homes'.Full time hours are preferable but please do not hesitate to get in touch if you would like to discuss your options.Healthcare Experience and experience of working with the frail populationis essential to this job role, ideally within Primary Care or a Community setting.

Main duties of the job This is an exciting and unique opportunity to be at theforefront of the delivery of our Frailty service and implementation of the PCNDES Enhanced Health in Care Homes (EHCH)APEX Primary Care Network (PCN) is seeking to recruit a phlebotomist/healthcareassistant with an interest in frailty to join our EHCH and Frailty team,combined of our Frailty Practitioner, senior Healthcare Support Worker and ourEHCH care co-ordinator and Clinical Care Co-ordinator in Frailty.This Role is based across 15 Care homes and 4 GP Practice's within APEX PCN, the successful candidate must be able to travel across all sites on a weekly basis.
About us Apex PCN is a four-practice network based in Lincoln with a largely urban to semi urban population.
We have a population of 46,000 with a predominantly younger demographic.
The PCN is supported by a strong ARRS team.
This includes pharmacists- engaged in supporting practices in their daily workload while also focussing on the DES requirements.
We have an effective FCP team supporting practices for MSK care and a well-developed EHCH MDT to support care homes.
Our practices work collaboratively to achieve our IIF targets and PCN DES requirements.
We are now looking at forming strategic partnerships with our other Health and Care colleagues to address effectively the health needs of our population.
Job description Job responsibilities -Undertake home visits for urgent and routine bloods tosupport the 4 practices of Apex- Working with residents andfamilies to complete Personalised Care and Support Plans-Support and assist other staff members to deliver care as delegated and directed- Completing baseline observations,and supporting the team with appropriate assessments for new residents- Reviewing care plans-Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role-To be involved in education forcare home staff- To be involved in local projectsrelevant to improving the health and wellbeing of care home residents withinthe Apex locality- Liaising with other services- Actively involved in the weekly MDT meeting Person Specification Experience Essential At least 2 years experience within a relevant Field.
