Job summary Trinity HealthGroup PCN is looking for a Clinical Pharmacist to join our current team of 5Clinical Pharmacist and 3 Pharmacy Technicians.
The post holder will work alongside theexisting team to support meeting the needs of our patients.
Main duties of the job The post holder will be undertaking structuredmedication reviews, improving medicine optimisation and safety, supporting carehomes, as well as running practice clinics, as set out in the PCN DES.
Part ofthis role will also include working within our Pharmacy Hub alongside thePharmacy Team to provide a safe and effective Pharmacy Service.
To provideprofessional specialist medicines advice to GPs, Nurse Practitioners, PracticeNurses, Community Pharmacists, Community Nurses, and other health professionalsto promote the safe, effective, and high quality, cost-effective use ofmedicines in all prescribing decisions for the benefit of the Practice'spatients.
You will also be actioning acute prescription requests and conductingmedicines reconciliation.
About us Trinity HealthGroup is a PCN made up of four practices on the west of the district (TrinityMedical Centre, Maybush Medical Centre, Crofton and Sharleston Medical Practiceand Warrengate Medical Centre).
The PCN team currently consists of ClinicalPharmacists, Pharmacy Technicians, Paramedics, Mental Health Practitioners,FCPs, Care Coordinators, Health & Wellbeing Coaches, GPAs and SocialPrescribing Link Workers.
