Job summary The post holder will -Bea pharmacist, who acts within their professional boundaries, supporting andworking alongside a team of pharmacists in general practice.The post holder will work as part ofa multi-disciplinary teamTakeresponsibility for areas of chronic disease management within the practice andundertake clinical medication reviews to proactively manage patients withcomplex polypharmacy.Toprovide primary support to general practice staff with regards to prescriptionand medication queries.Tosupport the repeat prescription system, deal with acute prescription requests,and medicines reconciliation on transfer of care and systems for saferprescribing, providing expertise in clinical medicines advice while addressingboth public and social care needs of patient in the GP practice (s).Provideclinical leadership on medicines optimisation and quality improvement andmanage some aspects of the quality and outcomes framework and enhancedservices.Toensure that the practice integrates with community and hospital pharmacy tohelp utilise skill mix, improve patient outcomes, ensure better access tohealthcare and help manage workload.
