Job summary INTERVIEWS WILL BE HELD ON MONDAY 16TH DECEMBERThe post holder will work within their clinical competence as part of a multi-disciplinary team to:provide expertise in clinical medicines managementprovide structured medication reviews with and for patientssupport patients in the management of their long-term conditionsoversee/support the management of medicines on transfer of care and implement systems for safer prescribingmanage repeat prescription authorisations and acute prescription requests.while addressing the public health needs of the registered population.The post holder will perform medication reviews for patients with polypharmacy, especially older people, people in residential care homes and those with multiple co-morbidities; this may be in a practice setting or via phone or video consultation as agreed with the practice and the Lead Clinical Pharmacist.The post holder will be involved in quality improvement and clinical audit as well as liaising with practices to manage some aspects of Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF) work.This role is pivotal to improving the quality of patient care and to developing operational efficiencies; it requires motivation and passion to deliver an excellent service within general practice.
Main duties of the job The post holder will work in accordance with specific practice guidelines and protocols where applicable.
He or she will be responsible for supporting network practices in implementing effective medicine management, identifying areas for improvement and initiating and managing change.
Achieving clinical and non-clinical targets and objectives as agreed with the PCN and practices will be part of the role.If you have not already, you will be required, and supported to complete the CPPE Clinical Pharmacist training pathway programme.

About us Havant & Waterlooville PCN comprises five GP practices:Bosmere Medical PracticeBlossom PracticeOaks HealthcareDenmead PracticeThe Village SurgeryThe Clinical Pharmacist is expected to work with and within the individual network practices as well as from home or within the community.The post holder will be part of a small Clinical Pharmacy team (currently two Clinical Pharmacists, plus the Lead Advanced Practitioner Clinical Pharmacist and a pharmacy technician), who work independently from each other for much of the time.
The CD with responsibility for clinical pharmacy is actively involved with the team and there is additional support, where required, from the PCN management team.Recognising that each GP practice operates differently, on a day-to-day basis the post-holder will engage with GPs and Practice Managers across the five practices in order to deliver the responsibilities of the role.

It is not an exhaustive list, and the post holder may be required to undertake other responsibilities and duties that are within his or her competence and which are appropriate to the role.The role and responsibilities are subject to review and revision from time-to-time and in discussion with the post-holder, if appropriate Person Specification Qualifications Essential Professional registration with the General Pharmaceutical Council Qualified from, enrolled or intention to enrol in an approved 18-month training pathway (eg CPPE GP, MOCH or PCN pathway) Be a prescriber, working towards or intending to become one within two years of start date Desirable Post-graduate Clinical Diploma Experience Essential Evidence of Continued Professional Development Minimum two years relevant pharmacist experience post-registration Good IT skills and literacy Experience of partnership/collaborative working and of building relationships across a variety of organisations Desirable Previous experience of working in primary care, in either a PCN or GP practice Experience of working with GP prescribing systems, other clinical systems, and Microsoft Office Experience of conducting medication reviews
