Job summary Location: CAM Medical Primary Care Network, CambridgeCAM Medical Primary Care Network is looking for a PCN Clinical Pharmacist to join our dynamic team working across three GP practices in Cambridge City.
Main duties of the job Candidates must be registered or in the process of registering with the General Pharmaceutical Council.
While primary care experience is advantageous, it is not mandatory.
Flexibility, enthusiasm, and a strong foundation in clinical knowledge are essential for success in this role.Successful candidates will receive support for clinical learning and professional development, including access to a GP clinical supervisor, collaboration with other clinical pharmacists, and enrollment in a formal education pathway provided by the Centre of Postgraduate Pharmacy Education (if not already completed).

Job description Job responsibilities Job SummaryThe clinical pharmacist will work across the CAM Medical Primary Care Network area in the pharmacy team.
Work undertaken at the practices will be determined according to their existing and developing systems and will include some or all of the responsibilities noted below: Medication Review and Optimisation Care Home Medication Reviews Long Term Condition Clinics Medicines Reconciliation and processing discharge letter Unplanned Admission Prevention Medicines Information / Education Prescribing Systems and Policies Medicines safety Liaison with community and hospital pharmacies Running reports and audits Any other duties as reasonably required.
Person Specification Experience Essential Primary care experience would be an advantage, however, is not essential.
