Job summary Cygnet PCN is seeking an enthusiastic Cancer Care Co-ordinator to join our GP practice network in High Wycombe.The PCN is made up of four GP practices, providing services for approximately 48,000 patients.
The practices have a strong history of working together and have been making significant strides towards integrated working.From 2028, 55,000 more people will survive five years following a diagnosis of cancer.
To achieve this, about 75% of cancer patients need to survive five years.
Currently just over 50% of patients survive 5 years.The aim of the Early Cancer Diagnosis deliverable is to support the early detection of cancers through improving our internal processes and supporting cancer screening uptake.
The aspiration for NHS England, is that by 2028, 75% of cancers will be diagnosed at an early stage (stage 1 or 2).
Currently just over 50% of patients are diagnosed at an early stage.We are looking for a candidate who can help us support this initiative and make a difference in improving screening uptake and cancer outcomes for our patients.This is initially a fixed term contract of 9 months, however, there is a potential to extend.
Flexible hours, term time hours etc will be considered for the right candidate.
Main duties of the job Review practice lists of patients screening uptake figure for the 3 National Screening Programmes; Breast, Bowel & Cervical.Support the practices to review the patient lists in terms of inequalities and demographics, such as learning disabilities, deprivation, etc.Run IT reports from EMIS, our GP System, to identify non-responders of screening.Capture and maintain cancer screening data within an Excel Spreadsheet, for all 4 practices.Work with the member practices, to contact non-responders of cancer screening.Investigate areas of improvement to support cancer screening uptake and share with the network.Work with other stakeholders, such as the Cancer Alliance, Bucks Health Trust and local authority partners, to improve the overall screening process and uptake.
About us The Cygnet Primary Care Network is an established Network serving 45,000 patients across 4 GP Practices in High Wycombe, a large, diverse town in Buckinghamshire.You will become a part of the administrative teams, working for all of the practices, supported by GP's, practice managers and other members of the practice and admin teams.There is a friendly atmosphere established at all sites, a long with a 'can do' work ethic.
There is a loyal and well established team in place, with a low staff turnover rate.All staff are invited to have an input into the development and smooth running of the PCN Job description Job responsibilities To assist the Primary Care Network (PCN) in delivering improvements to the services we provide our patients in relation to the following aspects of patient care.
Participation in national cancer screening programmes.
In support of collaborative working the post holder will form productive and supportive relationships with practice staff who have the skills, knowledge and remit to contribute to this work.
, GP practice non clinical cancer champions, social prescribers, pharmacists, secretaries etc.
The post holder will assist the PCN constituent practices to evaluate their screening uptake and engage underserved populations to reduce health inequalities.
This will include working alongside practices to enhance processes to track and follow-up screening non-responders.
Provide the PCN with support to host peer-to-peer learning events that look at data and trends in diagnosis and screening across a Network.
Including appropriate contributors from other organisations.
Source appropriate resources, training, system examples from appropriate organisation such as the Cancer Alliance, Cancer Research UK, Macmillan, and local authority partners.
Be a point of contact for PCN member Practices to develop and implement their cancer screening improvement action plans.
Create a Library of PCN data packs and other resources to support the delivery of information to patients in a wide variety of formats to meet the needs of all patient groups, including those with Learning Disabilities, and people for whom English is not a first language.
Review practice coding for report building and templates to ensure consistency across the PCN and accuracy of data.
Identify coding anomalies and liaise with Ardens (template and reports used by all member practices).
Provide administrative support for project evaluation and feedback Person Specification Skills Essential Ability to organise and prioritise workload effectively Ability to exercise sound judgement when faced with conflicting pressures Excellent record keeping skills IT literate Good verbal and listening skills Works well as an autonomous colleague and also within a team Excellent communication skills including able to communicate effectively, both verbally and in writing, with a wide range of people and stakeholders Excellent administration skills including able to prioritise and to finish work tasks Desirable Proficient in the use of web-based applications or programmes Awareness of local referral pathways Experience Essential Good experience of Microsoft Office applications Experience of data collection and providing monitoring information Desirable IT systems including EMIS Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).
Experience of supporting people, their families and carers in a related role.
Experience of collaborative working and building relationships across varied organisations Experience of working within a patient facing role Qualifications Essential NVQ Level 3, Advanced level or equivalent qualifications Demonstrable commitment to professional and personal development
