Job summary Tosupport the practice clinical team by signposting patients to the appropriatehealthcare professional or service, working as part of the practicesmultidisciplinary team.
Main duties of the job The following are the core responsibilities of the care navigator.
There may be, on occasion, a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels.
a.
Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition ensuring the patient gets the right support b.
Provide administrative support to Safeguarding leads including attendance at multi disciplinary meeting, patient related paperwork, safeguarding follow ups, minuting c. Keep members of the practice up to date with any new/existing safeguarding concerns or patients with additional vulnerability d. Initiate contact with and respond to requests from patients, team members and external agencies e. Enter read/SNOMED CT code data f. Input data into the patients healthcare records as necessary g. Manage all queries as necessary in an efficient manner h. Maintain a clean, tidy, effective working area at all times i.
Undertake all mandatory training and induction programmes j.
Attend a formal appraisal with their manager at least every 12 months.
Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed k. Action incoming emails when necessary b. Scan patient-related documentation and attach scanned documents to patients healthcare records c. As required, support the reception and administrative team as delegated by the management team Person Specification Experience Essential Experience Experience of working with the general public Experience of administrative duties Desirable Experience of working in a healthcare setting Experience of safeguarding processes Qualifications Essential GCSE English (C or above) and at least three others Desirable Educated to A-level/equivalent or higher, NVQ Level 2 in Health and Social Care
