Job summary Clinical Lead Patient Flow - Band 7Fixed Term - Maternity CoverChippenham Community HospitalWe are recruiting to the positionof Clinical Lead based within the Wiltshire Patient Flow Hub at ChippenhamHospital for a fixed term to provide maternity leave cover.
The position isavailable from March 2025 and any seconded position will need to be agreed withtheir existing line manager and HCRG before commencing.
Wiltshire Health andCare services are transferring to HCRG from 1st April 2025.
** Interviews will be held on 4th February **The role involves working to deliver on allaspects of patient flow within Wiltshire Health and Care.
This includes takingand processing referrals for onward care, liaising with acute and non-acutehospitals and community based teams.
You will need to have a good knowledge ofthe health and social care criteria and the differing services which areavailable.
Working knowledge of the national discharge framework pathways isalso important.
Main duties of the job You will be a registered clinicalpractitioner with experience in a relevant field.
You will be able todemonstrate a good understanding of the patient journey and an understanding ofhow community services support patients.You will have good communication skillsand be able to negotiate with staff from a wide range of backgrounds.
You willshow attention to detail and excellent record keeping.
High level digitalskills are required as all work is computer based.
The WiltshirePatient Flow Hub is a team of clinical and admin staff based at ChippenhamHospital.
We work closely with our acute partners, our inpatient wards,intermediate and social care providers and community teams.

Person Specification Qualifications Essential Educated to post graduate level or to have equivalent knowledge and experience Management, business or clinical Qualification or working towards one Desirable Evidence of development of leadership skills (accredited) Additional Criteria Essential Access to a vehicle that can be used for business purposes and hold a full and valid driving licence.
Experience Essential Evidence of successfully leading a team Experience of resource management Experience of managing HR issues Experience in delivering agreed change agendas Experience in delivering clinical governance, risk and safety management Experience in undertaking investigations into complaints Proven ability to work with colleagues across organisations.
Desirable Experience of leading a project.
Experience in undertaking investigations into clinical incidents Proven ability to work with colleagues across health and social care.
