Job summary Our Patient Flow and Discharge Co-ordinator will be a fundamental member of the Trustwide Site operational team to manage patient flow and discharge.
They willmanage patient flow throughout the urgent and emergency care system in order to achieve the quality standards and to provide support to multidisciplinary teams in achieving this goal.
They will also oversee the trust discharge lounge operationally and manage patient flow for the elective pathway to provide a single point of contact on a named ward for patients, families, carers and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinarycolleagues.
They will liaise effectively with professionals throughout the healthcare system in maintaining patient flow and ensuring live bed state information with existing PAS system, Swiftplus and nerve centre, manage community beds, ensuring fair and equitable allocation to community beds across Torbay and South Devon and ensure patients are repatriated in a timely manner as detailed in the Repatriation Policyacross the ICB and wider NHS Responsible for the management of private transport requests and management of the out of hours.
They will compile and report daily Trust performance updates and sitreps to NHS England/Improvement and commissioners.
