Job summary Here in the Discharge to Assess Pathway 1 Team we have fantastic opportunities for therapists looking for the next step in their career.We have 4 small friendly teams across Lincolnshire comprising of Occupational Therapists, Physiotherapists, Nurses, Pharmacy Technician, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants.
We are a growing team and looking for enthusiastic, friendly, and proactive therapists.In our Pathway 1 Team, we follow the patient home and see them within 2 hours of their discharge.
We listen to the patient of what matters most to them with regard to their personal and social circumstances, their physical, mental and emotional health and their carer's needs.
This will include exercise programmes for their physical and mental wellbeing, recommending and providing the correct equipment, proactive work with patients to encourage independent living, educating patients on self-management of their health and wellbeing such understanding frailty symptoms and the importance of staying well physically and mentally for falls prevention, and so much more.
Main duties of the job Being part of our Team, you will be working closely with colleagues across all sectors when supporting patients in the community ensuring a high standard of therapy provision.

Person Specification Qualifications Essential Degree or equivalent postgraduate diploma in Physiotherapy/ Occupational Therapy Current Health Professions Council Registration Evidence of relevant postgraduate training Evidence of continuing CPD Basic IT skills Desirable Member of Chartered Society of Physiotherapy or RCOT Previous experience Essential At least 2 years experience as a physiotherapist/ occupational therapist Experience in working in the NHS Evidence of leading in areas of team working and/or development Desirable Experience in multi-disciplinary working Experience of working in Community and Acute Knowledge/skills/aptitude Essential Good communicator in a variety of levels and methods Understands Discharge to Assess Desirable Supervision of junior, students and unqualified staff
