Job summary In recent years the role of General Practice and Community Care has expanded at an exponential rate.

We are seeing ever more complex patient care needs and targets.
We are recruiting to an experienced Paramedic Practitioner to be part of our PCN Hub Team.
Paramedic Practitioners are a highly valuable additional to the Primary Care Team.
Folkestone, Hythe and Rural PCN wishes to integrate Paramedic Practitioners into our dynamic workforce, this is an exciting opportunity to be part of our highly organised, friendly and progressive team.
We already have paramedic practitioners within our team which will offer our successful applicant opportunity for orientation and supervision as required.The post holder will be required to travel across the PCN locality and therefore will need to hold a driving licence and be in possession of their own car.
Main duties of the job Personal SpecificationEssentialCompletion of undergraduate degree/diploma in Paramedicine or equivalent experience.
Working at Level 7Registration with Health and Care Professions Council.Completion of two-year Consolidation Of learning period as a newly qualified paramedicHas a further three years experience as a band 6/7 (or equivalent) paramedicDesirableCompleted or working towards developing level 7 capability in paramedic areas of practiceIndependent prescribing Qualification or steps towards thisFurther Post Graduate Qualifications or DiplomasExperienceEssentialFour years or longer post paramedic qualification including 3 or more years at band 6 levelDesirableWorking knowledge of General PracticeExperience of supporting Care Home ResidentsExperience of Minor Illness managementExperience of Primary Care IT Systems About us Folkestone, Hythe and Rural Primary Care Network (FH&R)is an innovative, friendly and forward thinking group.

We are proud and enthusiastic to be an inventive front runner in the new era of The Primary Care Network Job description Job responsibilities Tobe responsible for the provision of paramedical services, working as part ofthe primary care networks multidisciplinary team, delivering care within their scope ofpractice to the entitled patient population.
The paramedic practitioner will berequired to work autonomously as part of the team, providing same day response to unwell care home residents, and scheduled minor illness clinics (either via telephone or face to face appointments, they may be required to undertake home visits as we develop our home visiting service.
The successful applicant will lead regular Care Home rounds, providing proactive clinical care to identified residents.
The role provides a range of services such as assessment, differential diagnosis andtreatment, referring patients appropriately, as well as supportingthe management team in the reviewing of clinical policy and procedure.Thefollowing are the core responsibilities of the paramedic practitioner.
Theremay be on occasion a requirement to carry out other tasks; this will bedependent on factors such as workload and staffing levels.
The paramedic practitionerwill work within their scope of clinical practice and:Identify,signpost or refer patients at risk of developing long-term conditions,preventing adverse effects on the patients healthProvideroutine care to patients as required in accordance with clinically basedevidence, NICE and the NSFExamine,assess and diagnose patients and provide clinical care/management as requiredTriagepatients and provide the necessary treatment during care home/home visitsCollectpathology specimens as requiredMaintainaccurate clinical records in line with extant legislationEnsurethat read codes are used effectivelyEnsurecontinuity of care, arranging follow-up consultations or reviews as necessaryReviewthe effectiveness of the treatment provided, making changes where necessary toimprove patient outcomesChaperonepatients where necessaryPrioritisehealth issues and intervene appropriatelyActas the lead clinician when dealing with clinical emergenciesRecognise,assess and refer patients presenting with mental health needsSupportpatients in the use of their prescribed medicines or over-the-counter medicines(within own scope of practice)Liaisewith external services/agencies to ensure that the patient/care home issupported appropriately (vulnerable patients, etc.
)Adhereto the relevant patient group directives and local clinical pathways at alltimesSupportthe clinical team with all safeguarding matters, in accordance with local andnational policiesUnderstandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriatelyDeliveropportunistic health promotion where appropriate Person Specification Qualifications Essential Registered Paramedic (HCPC) Postgraduate diploma or degree (primary care) Member of Health and Care Professions Council Completion of a minimum of 4 years post qualification experience, with at least 3 years at Band 6 Working towards developing Level 7 capability in paramedic areas of practice and has completed and been signed off formally within the clinical pillar competencies of the Advanced Clinical Practice Framework.
Desirable Non-medical prescribing qualification (or willingness to work towards) Experience Essential Experience of working as a paramedic practitioner Experience in triage Experience of dealing with a range of clinical conditions Desirable Experience of working in a primary care environment
