Job summary:The role will provide a Paramedic Practitioner or Nurse Practitioner resource for health care professionals andservice users, working in collaboration with other members of theMultidisciplinary Team.To work within the community,as an autonomous, accountable, Specialist / Advanced Paramedic, in theprovision of a holistic approach for individuals including assessment,management and treatment, to deliver high quality patient services.To assess, manage, treat,refer and/or signpost patients/service users who attend surgery withundifferentiated or undiagnosed condition relating to minor illness or minorinjury.The post holder will useadvanced clinical skills to provide education to service users, promotingself-care and empower them to make informed choices about their treatment.Job responsibilities:Housebound home visit requestsManaging and treating patientspresenting with minor illnessTriage and treat patientswishing to see a health care professional, making any necessary referrals toother members of the primary health care teamPatients with problems needingreferral to secondary care should be discussed with the registered GP beforemaking such referralEnsure clinical practice issafe and effective and remains within boundaries of competence, and toacknowledge limitationsAdvise patients on generalhealth care and minor ailments, with referral to other members of the primaryand secondary health care team as necessaryWork from the surgery andwithin communities as an autonomous practitioner caring for patients andfamiliesWork as an autonomouspractitioner, in accordance with the Health and Care Professions Council.Ensure that personal andprofessional clinical standards are maintainedUndertake assessment forpatients within the community and those attending the surgery, using diagnosticskills and initiation of investigations where appropriatePrescribe/issue medications asappropriate following policy, patient group directives NICE (national) andlocal clinical guidelines and local care pathways.Formally and informally impartknowledge and skills to colleagues promoting peer review and best practicewithin the work environmentCommunicate at all levelswithin the team ensuring an effective service is deliveredMaintain accurate,contemporaneous healthcare records appropriate to the consultationEnsure evidenced-based care isdelivered at the highest standards ensuring delivery of high-quality patientcareWork within local policies andproceduresEnhance own performancethrough Continuous Professional Development, imparting own knowledge andbehaviours to meet the needs of the serviceAchieve and demonstrate agreedstandards of personal and professional development in order to meet the needsof the serviceParticipate in the auditprocess, evaluation and implementing plans and practice change in order to meetpatient needActively participate inmentoring and supervision, plus practice-derived CPD opportunitiesContribute positively to theeffectiveness and efficiency of the team and work colleaguesPathological specimens andinvestigatory proceduresUndertake the collection ofpathological specimens including intravenous blood samples, swabs etc.
Performinvestigatory procedures needed by patients and those requested by the GPsAdministration andprofessional responsibilitiesParticipate in theadministrative and professional responsibilities of the practice teamEnsure the clinical computersystem is kept up to date, with accurate details recorded and amendedEnsure appropriate items ofservice claims are made accurately, reporting any problems to the practiceadministratorEnsure accurate completion ofall necessary documentation associated with patient health care andregistration with the practiceEnsure collection andmaintenance of statistical information required for regular and ad hoc reportsand auditAttend and participate inpractice meetings as requiredTraining and personaldevelopmentTraining requirements will bemonitored by yearly appraisal and will be in accordance with practicerequirements.Complete the PCN road map course locally.
Time and support will be given.Personal development will beencouraged and supported by the practice.
It is the individuals responsibilityto remain up to date with recent developments.Participate in the educationand training of students of all disciplines and the introduction of all membersof the practice team where appropriateMaintain continued educationby attendance at courses and study days as deemed useful or necessary forprofessional development, ensuring PREP requirements are metIf it is necessary to expandthe role to include additional responsibilities, full training will be given.Develop and maintain aPersonal Learning PlanLiaisonWork closely with themulti-professional, management and administration teams to ensure the smoothrunning of the practice, reporting any problems encountered to the relevantperson and ensuring everyone is aware of the different roles within the teamThere is also the need toestablish and maintain good liaison with other surgeries and agencies,including secondary and social careMeetings:It will be necessary to attendand contribute to various practice meetings as requested.

Person Specification Qualifications Essential Qualifications BSc in a training programme approved by the College of Paramedics Health & Care Professions Council (HCPC) registration able to operate at an advanced level of clinical practice Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11 Pre-reg MSc in a training programme approved by the College of Paramedics Nurse Practitioner Full UK driving license Desirable Pre-reg MSc in a training programme approved by the College of Paramedics non-medical prescribing qualification Experience Essential Essential Knowledge experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans working knowledge of Microsoft and GP practice and prescribing data monitoring systems ability to write comprehensive clinical notes, implement and evaluate care plans working towards advanced clinical practitioner status mentorship or supervisory skills training ability to undertake nails surgery assessments, procedure and post-operative care be aware of data protection (GDPR) and confidentiality issues particularly within a PCN cognitive behavioural and motivational interviewing approaches / skills Analysis skillsability to evidence a sound understanding of the NHS principles and values ability to analyse and interpret complex/ often incomplete information, pre-empt and evaluate issues, and recommend and appropriate course of action to address the issues experience of working within a primary care setting evidence of working across organisational boundaries within health and social care independent thinker with good judgement, problem-solving and analytical skills Desirable Desirable Experience of general practice and/or secondary care Any desirable/additional specialty specific requirements
