Job summary South Tyneside andSunderland Paediatric Bladder and Bowel Team have an exciting opportunity for aRegistered Childrens Nurse with significant experience of working in apaediatric area, to join our team.
Due to a recent structurechange, this post now sits under the Paediatric directorate.
We are lookingfor a Band 6 clinician who is filled with passion to become part of our service.Someone who possesses excellent communication skills with an ambition tocontinually develop the service to improve the patient/family journey.
The post holderwill be required to undertake holistic Tier 2 level continence assessments, andwork with children and families to develop individualised treatment programmesto promote continence.
The post holderwill hold clinics in both Sunderland and South Tyneside and deliver trainingprogrammes to STSFT staff.Informal visitsencouraged.

Main duties of the job Team workinghours Monday Friday Service covers South Tyneside and Sunderland.To adopt aphilosophy of continued professional development in line with service provision,working within the scope of Specialist Nurse Role following localpolicies/protocols /guidelines to meet the needs of children and young peoplewith continence problems.

Job description Job responsibilities Please see attached Job Description for full detailsTo supportin the day-to-day management of an integrated evidence-based CommunityPaediatric Bladder and Bowel Service with responsibility for the delivery ofpatient care optimising the cost-effective use of all resources.
To supportcoordination of paediatric Bladder and Bowel service provision withinSunderland and South TynesideTo managea caseload for the children bladder and bowel service working at Tier 2 levelTo workwith paediatric colleagues in providing an accessible and multi disciplinarycontinence service.Ensure aseamless service and help reduce inappropriate referrals to secondary care byworking in collaboration with tier 2 paediatric staff.To respondeffectively to the childs physical, psychological and social needs by workingalong-side Secondary care, Education, Children and Young People Service andSocial Services.Totransform service delivery by enhancing collaborative working across hospitaland communityTo meetthe needs of all children and young people with continence problems (includingchildren with learning difficulties and physical disabilities) in order toachieve complete continence or manage their condition discreetly andeffectively if full control is not possible.To work inappropriate care settings, within the scope of a specialist nurse rolefollowing local policies/protocols/guidelines.To assessindividual childrens needs using a comprehensive bladder /bowel assessment,determining a plan of care and implementing holistic, research-based healthcare (including initiating investigations and prescribe medication whennecessary)Work incollaboration with tier 1/2/3/4 services in bowel and bladder care to providehigh quality care which follows best practice and meets the NHS outcomeframework domains (1, 2, 3, and 4).Support toensure the service meets NICE guidelines, Department of Health and NHS Englandrecommendations.Triage allreferrals into tier 2 Paediatric Bladder and Bowel service ensuring that localguidelines are followed, and referral is to the appropriate professional.To helpmanage the provision of NHS disposable containment products for children inSunderland and South Tyneside To act asa resource of specialist Paediatric continence advice in Sunderland and SouthTyneside by providing training and support to internal and external staff.Tofacilitate and support the development of a communication network forPaediatric continence advice and information for staff.To ensuresystems and processes are in place to maintain patient safety.To supportthe development of user involvement within the Paediatric Bladder and BowelService to improve the child/family journey.Supporttransition of care into adult Bladder and Bowel services Person Specification Skills & Knowledge Essential Registered Childrens Nurse with significant experience of working in a paediatric area Degree in nursing/health related subject.
