Job summary We are looking to recruit a Nurse or Physiotherapist to Home Oxygen service in Worcestershire.The role is community based, seeing patients in their own home, performing LTOT and AOT assessments on patients referred from GP or as step downs from hospital.We are looking for a enthusiastic and passionate individual, who is confident with oxygen prescription, and is able to work confidently as a lone worker.
Main duties of the job The overall purpose of this role is to co-ordinate the HOS-AR service for Worcestershire, ensuring that the key expected outcomes outlined in the service specification are delivered.
These include provision of a systematic, responsive, integrated, efficient HOS-AR that ensures service users have a positive experience of care.
The scope of the service is adult patients with a Worcestershire general practitioner.

Job description Job responsibilities Key DutiesTo develop clinically effective practice in the HOS-AR service through care pathways, which are integrated alongside the whole system of primary and secondary care.1.
Provision of Specialist Clinical ServicesThe post-holder will: To triage all referrals for oxygen assessment, allocating the patient to the most appropriate assessment clinic.
To provide a responsive, integrated, easily accessible assessment and follow up service To carry out specialist oxygen assessments of adult patients in both acute and non-acute situations.
This procedure may include: pulse oximetry, arterial and capillary blood gas sampling, exercise testing and assessment, functional assessment and measurement of psycho-social status.
Where appropriate, it may be necessary to deal with other aspects of the patients during oxygen assessment/review.
This may include managing acute exacerbation, nebulised medication, breathlessness, panic and fear, patient monitoring, education of patient and carers, and link into other appropriate health care professionals, for example neurology and cardiology.
To identify accurately those patients who will benefit from Long term Oxygen (LTOT) And review of existing patients to identify those who have prescribed home oxygen but will not benefit Patients who retain CO2, consider non-invasive ventilation (NIV) with advice from Respiratory Physician as required To ensure all assessments and reviews are undertaken in accordance with the British Thoracic Society Guidelines Plan, deliver and evaluate the care of patients in partnership with other hospital and community teams.
To develop oxygen assessment and review pathways for patients in community settings, both the patients home and community institutions.
To construct and maintain a countywide oxygen database with IT assistance.
To use communication tools (verbal and non-verbal) to gain informed consent from patients including where there are barriers to effective communication.Using appropriate assessment tools ensure an appropriate level of nursing intervention so that patients who present with additional or more complex needs are referred to the appropriate clinician.To discuss all treatment options with sensitivity, knowledge and expertise and to act as patient advocate when appropriate and respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements.To work in partnership with the patients to empower them to make informed choices about their health and their healthcare.Establish and maintain communication links with other professionals while dealing with complex and sensitive information.Communicate with patients and carers using a range of communication skills to manage care and information which may be of a complex, and distressing nature dealing empathetically with sensitive information, including discussing patients changing condition with patient and/or carerKeep accurate and up to date records in partnership with clients.Actively respect patients privacy and dignity and protect their property and possessions.2.Education and Training Participate in the Trust personal development review (PDR).Agree service and personal objectives with the Clinical Team Lead and be responsible for self-audit to demonstrate both achievement of objectives and evidence of value to the trust.Provide specialist advice, teaching and training of other healthcare professionals with regard to optimal management of patients with COPD on a formal and informal basis.Advise, educate and train other members of the team, as appropriate, to ensure high quality clinical care is delivered to patients.Commit to a philosophy of care which has an emphasis on self-management and patient empowerment through undertaking health education and health promotion activities.To be an active member of the in-service training programme by attending and delivering presentations at staff meetings, tutorials and training sessions internally and by attending external courses and utilising reflective practice to ensure continuing professional development.To actively participate in audit and/or research, leading in specific projects and supporting others.
Thus facilitating evaluation of services offered, quality of care and further developing the services delivered by the HOS Team in conjunction with the Clinical Team Lead.Act as a clinical educator for students within the team providing the appropriate level of clinical supervision, education and assessment according to the training provider standards and regulations.2ProfessionalBe professionally and legally accountable for all aspects of own work; including caseload management.

ManagerialTo assist in the maintenance and update of protocols according to national and local guidelines for the safe and effective provision of the HOS-AR serviceWork as an effective member of the department of thoracic medicine, participating in multi-disciplinary working initiatives and contributing to the development of organisation change.Provide monthly reports to commissioners and GP practices.To contribute to the ongoing promotion and marketing of the HOS-AR service to all key stakeholders including patients/carers.To organise the monitoring, evaluation and audit of the HOS-AR service quality and develop reporting mechanisms in order to meet the locally set targets outlined in the service Specification.To be responsible for ensuring sound governance within the HOS-AR teamTo attend and chair home oxygen meetings, and ensure action, as appropriate.Along with other senior team members, to develop effective teamwork, cohesion and collaborative working.Along with other senior team members, to use evidence, vision and creativity to design and develop the service for the future.Participate in the departmental service development strategy and promote its implementation within practice.Utilize and keep up-to-date with administrative systems to support work programmes.Be responsible for all aspects of the line management of band 6 practitioners.
