Job summary WestburyGroup Practice is a growing, forward thinking GP practice in the heart ofWiltshire, 30 minutes from Bath.
We are seeking a motivated and enthusiasticregistered Older Person's Nurse to be part of our living well team.
Main duties of the job You would be assessing, planning, implementing and evaluating the care of older people with complex needs in the community, therefore a driving licence is essential.
It is a varied role, incorporating home visits to promote health and independence, prevent hospital admission and support advance care planning.
You will be working closely with Practice GP's as well as the clinical care coordinator and social prescriber.

The role will primarily be based at White Horse Health Centre in Westbury.

Refer patients as appropriate to other members of the MDT, secondary care and other statutory and voluntary organisations.
Make clinical judgements appropriate to the expectations and demands of the job.
Offer a holistic service to patients and their families/ carers in conjunction with the wider multi-disciplinary team; developing where appropriate and on-going plan of care/ support, with an emphasis on prevention and self-care Complete memory assessments Complete frailty and dementia reviews Respond to sudden changes in health or social circumstance Present complex issues at the multi-disciplinary meetings and chair if necessary Be aware of, and contribute to planning and delivery of service, promoting integrated and seamless pathways of care.
Work and adhere to evidence based practice, recognised clinical guidelines and practice policies.
Advise, support and mentor other members of the clinical team.
Effectively manage own time, workload and resources.
Identify safeguarding concerns and make referrals as appropriate Administration and professional responsibilities: Participate in the administrative and professional responsibilities of the practice team Ensure accurate and contemporaneous notes of all consultations and patient contacts are recorded.
Ensure QOF templates are completed accurately.
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice Attend and participate in practice/nursing meetings as required.
Person Specification Knowledge and Skills Essential Frailty Dementia care Falls prevention Advanced care planning Working knowledge of the mental capacity act Ability to form management plans for patients with acute & chronic conditions & complex needs Decision making skills Prioritising care Communication skills, both written and verbal Able to use IT based clinical record system Time management and ability to prioritise workload Desirable Knowledge of primary & community health care Local & national health policy and impact on General Practice Awareness of chronic disease management and pathways Use of GP clinical record system, SystmOne Clinical leadership skills Qualifications Essential NMC registered nurse Current UK driving licence and insurance Desirable Evidence of post graduate study Teaching and mentoring qualification Experience Essential Minimum 2 years post registration Recent experience in care of older people in either the primary or secondary care setting Experience of a multi-disciplinary team working Experience of caring for people with dementia Ability to evaluate safety & effectiveness of own practice Desirable Involvement in mentoring/teaching learners Experience in change management and service development Working with people who self-neglect
